and recommendations made from the research conducted.

THE PROPOSAL

I. VISION AND MISSION (SCT’s Vision and Mission)

As a member of the Dominican- Siena School System, Siena College of

Taytay commits to the evangelizing mission of the Church through

integral faith formation that promotes academic proficiency and values

lived by Mother Francisca del Espiritu Santo de Fuentes.

By 2026, we envision our school to be a Center of Excellence

II. PROGRAM DESCRIPTION

The Perception of the Medical Staff on the Efficiency of Medical Record

Management in Cainta Municipal Hospital is a study that aims to find the efficiency

of manual medical recording in terms of time efficiency and safety.

As the study was conducted and as the data were collected, the researchers

found out that for the Cainta municipal Hospital, manual medical or paper based

recording is effective and efficient for their public service.

In line with the results and findings, the researchers recommend to

furthermore improve the Hospital's medical record management for more

effectiveness and more efficiency in time and in record keeping safety for the benefit

of the Cainta municipal Hospital community, most especially for the benefit of the

patients.

The study’s recommendation then was used by the researchers to shape and

construct a useful project proposal entitled “Improvement of Medical Record

Management: Implementation of Electronic Medical Records System”. The project

aims to provide better services and better medical record keeping for a much

improved healthcare service.

With the help of this proposed project, the medical record keeping in Cainta

Municipal Hospital may become more effective and efficient most especially in

terms of time efficiency and safety that will ensure that the needs of the patients are

provided for in a much more immediate response with a much more secured

recording of their medical information, which may result to an excellent health care

service.

III. OBJECTIVES

An Electronic health record is an official record which digitalizes the health

information for improving efficiency, quality of care and it reduces the costs. In other

words, Electronic Health Records are recorded, stored, and accessed on a network

which may include multiple medical providers. It would allow us to meet our goal of

having a completely paperless medical record. The objectives of this project are to

provide accurate, up-to-date, and complete information about patients at the point of

care, to have quick access to patient records for more coordinated, efficient care, to

improve patient and provider interaction and communication, as well as health care

convenience, to enabling safer, more reliable prescribing, to improve the productivity

of the medical staff and to enabling providers to improve efficiency and meet their

business goals.

IV. PERSONS INVOLVED

According to Bourgeois, D. (2019) The Creators of Information Systems is

first group of people that the company is going to look at to play a role in designing,

developing, and building information systems. These people are generally very

technical and have a background in programming and mathematics. The following

are the part of Creators of Information Systems.

Systems Analyst- The role of the systems analyst is to straddle the divide between

identifying business needs and imagining a new or redesigned computer-based

system to fulfill those needs. The analyst will identify the different stakeholders in

the system and work to involve the appropriate individuals in the process.

Programmer- spends their time writing computer code in a programming language.

In the case of systems development, programmers generally attempt to fulfill the

design specifications given to them by a systems analyst.

Hardware engineer- a hardware engineer designs hardware components, such as

microprocessors. Hardware engineer’s job is to write code to create a program that

will be implemented directly on a computer chip.

Software engineer- software engineers do not actually design devices; instead, they

create new programming languages and operating systems, working at the lowest

levels of the hardware to develop new kinds of software to run on the hardware.

Systems engineer- takes the components designed by other engineers and makes

them all work together. For example, to build a computer, the mother board,

processor, memory, and hard disk all have to work together. A systems engineer has

experience with many different types of hardware and software and knows how to

integrate them to create new functionality.

Network engineer- understands the networking requirements of an organization and

then design a communications system to meet those needs, using the networking

hardware and software available.

V. PROCESS OF IMPLEMENTATION

INPUT

The researchers found out that the perception of the Medical Staffs on the

medical record management in Cainta Municipal Hospital is that it is efficient in

terms of its time it consumes and its safety. After the result that was gathered, the

researchers would like to recommend some improvement to the medical record

management of Cainta Municipal Hospital.

PROCESS

The researchers would like to improve its medical record management

through upgrading the paper record management into digitalized record data where

it will need the installation of Information System to the Hospital.

OUTPUT

The expected outcome of the researchers in this project is that the Cainta

Municipal Hospital will have an improvement in terms of its medical record

management and it will help the work of the medical record staff to be more

convenient.
