6 hours of humanities (government, history, foreign language, art, social science and religion)
6 hours of Chemistry (general or higher) Intro to Chemistry will NOT count. Must be within 8 years of
starting school, preferred within 5 years (NLT Fall 2012 for FY19 application) and lab is not required but
highly recommended.
3 hours of Human Anatomy (can be a combined A&P I class) Must be within 8 years of starting school,
preferred within 5 years (NLT Fall 2012 for FY19 application) and lab is not required but highly
recommended.
3 hours of Human Physiology (can be a combined A&P II class) Must be within 8 years of starting school,
preferred within 5 years (NLT Fall 2012 for FY19 application) and lab is not required but highly
recommended.
Medical Terminology course within 5 years (NLT Fall 2015 for FY19 application) - can take on ALMS
SAT with ESSAY required within 5 years of application with a minimum score of 450 in reading and math.
A cumulative score of 1000 is required in the reading and math sections. (NLT March 1, 2014) Take SAT
prior to 1 January to allow 8 weeks for the College Board to grade your exam. Enter code "3994" on SAT
test Form.
A minimum GPA of 3.0 is required in all sciences with and minimum cumulative GPA of 2.5. The science
GPA will include science courses completed within 10 years of the start of IPAP (prior to Fall 2010). See
"Science courses for GPA" excel document under application info below.

performance will be needed.
DD 2807-1 & DD 2808 (include original copies of Urine drug and alcohol (ETOH) blood testing,
HIV, urinalysis, CBC, HCG (for females), as well as Audiogram)
- Include ORGINAL COPIES OF LAB PRINTOUT for the following labs: Urine drug screen
(UDS), alcohol blood test (ETOH), urinalysis (UA), HIV, CBC, HCG (for females)
- Include printout of Audiogram
- DD 2808 needs final signature from an MD or DO
- PHA WILL NOT SUFFICE!!! Everyone needs a physical completed after June 2018
