    1. I understand that refund of application / amendment / acceptance fee will be made to proven cases only.
    2. I agree that the refund, if approved, will be credited either to the original credit card account or the bank account provided
    in Part III above, as appropriate.
    3. I declare that the bank account information entered in Part III above is correct. If by mistake I enter the wrong account
    information, I will not be able to receive the refund and the JUPAS Office will NOT give me another refund.

    Notes:
     This form must be completed and returned to the JUPAS Office either by email, post or fax by 31 August 2020. Request for refund
    received after the above deadline (i.e. 31 August 2020) will NOT be entertained.
     Refund of amendment fee will be processed after the deadline for updating of programme choices (i.e. 20 May 2020).
     Copy of HKID card MUST be provided for processing refund.
     Copies of documentation showing bank account information (i.e. account name and account number) MUST be provided to facilitate the
    refund.
    Should there be any discrepancy between the information you have provided in this form and the bank account information shown in the
    supporting documentation, the latter will be considered as the account to be used for the refund.
