Job Description Hazard Identification Hazard Controls


• Ensure work area is clear of all non-essential personnel.
LOCATE • Spotters shall be used to protect surveyor from impact
from moving equipment.
ALIGN • Struck By • Heavy equipment operators and other trade site
• Caught Between personnel shall be informed, before the surveyor begins
OBTAIN ELEVATIONS • Run Over working, of his presence on site.
• Surveyor shall wear a hard hat and hi-visibility vest.
• The job site perimeter fence will eliminate exposure to
pedestrian or vehicular traffic.

Employee Instructions:
1. Hard hats, safety glasses, hi-visibility vest, work clothes, and work boots shall be worn at all times.
2. This JHA must be reviewed with all site personnel, before the surveyor begins to work.

All crew members must print/sign their name below to acknowledge their understanding of the JHA.
