First American NCS Office will pay a 32% split of the Premium and any endorsements to Policy-Issuing
Agent.
The Issuing Agent has documented search fees, update fees, copy fees and cancellation charges on this
form based on the information provided with the original order. Material changes to the Order may cause
these fees to be adjusted. These order changes include, but are not limited to, property location,
property description, coverage, product, liability amounts and endorsements.
Cancellation Charges:
Please provide your preliminary statement of costs for the title search in the event of cancellation.
Invoicing:
Please Note: Invoices must be sent directly to NCS and the State Agency Office, please do not
forward any invoices to the client.

Along with the commitment to insure the interests as indicated above, NCS must be provided with 1 SET(S) OF
LEGIBLE COPIES AND ALL CROSS REFERENCED DOCUMENTS of the following items:

Kiley Cermak, Amanda Angstman, if delayed beyond requested due date. Amount – 1000 ( this is just a
placeholder amount. WE don’t have an exact loan amount yet).
1. NCS File Number NCS-982941-MPLS must appear on Title Commitment and any correspondence.
2. All documents mentioned in the Schedule A (or Schedule C) legal description and all
underlying documents inclusive of plats listed on Schedule B, Parts I and II;
3. Source deeds (or Memorandum of Lease, where appropriate);
All municipal searches that are available; i.e. taxes, assessments, water, sewer, judgments and

any local searches that are customarily performed;
5. Information regarding the tax payment periods and the date when taxes become delinquent.
6. If transaction is a Leasehold Transaction, the following Requirement must be added.
In connection with that certain lease agreement dated (the "Lease"), as evidenced by that certain
Instrument No. in the Public Records, the Company requires: (x) a duly authorized estoppel executed
by the landlord stating (i) that the Lease is in full force and effect, (ii) that there are no defaults or
violations under the terms of the Lease, (iii) that all lease payments due and owing the landlord have
been paid current, (iv) that neither party to the Lease has any Known claims against the other under
the terms of the Lease, and (v) that there are no amendments or modifications to the Lease other than

as set forth in the estoppel, and (y) a dual authorized consent by the landlord, which may be included
in the estoppel described above, to the transaction creating the estate, interest or Mortgage to be
insured by the policy, if such consent is required under the terms of the Lease.
