    A RESOLUTION APPOINTING MRS. AMELIA M. TAMAYAO AS GENERAL MANAGER OF


    PEÑABLANCA WATER DISTRICT (PWD) EFFECTIVE __________.

    WHEREAS, the Board of Directors decided to approve the re-organization of the board and
    upon the acceptance of Engr. Calixto C. De Leon to relinquish his position as General Manager of
    PWD, the Board of Directors declared the said position vacant;

    WHEREAS, with the desire of the Board to ensure the continuation of the implementation of
    the on-going projects and improvements of the existing area of PWD and to further the projects of the
    same in the future, the Board of Directors of Peñablanca Water District on ______,conducted a
    thorough evaluation for the individual officer’s job descriptions more particularly the position of General
    Manager following the relinquishment of term Engr. Calixto C. De Leon in the said position;

    WHEREAS, Section 23 of PD No. 198 otherwise known as Provincial Water Utilities Act of
    1973 as amended by Section 9, PD 768, RA 9286 expressly states that “…the board shall appoint, by
    majority vote, a general manager and shall define his duties and fix his compensation…”;

    WHEREAS, upon recommendation and thorough evaluation, and in taking recourse to the
    provisions cited above, the majority of the Board of Directors of Peñablanca Water District have agreed
    and voted to appoint Mrs. Amelia M. Tamayao, a member of the Board of Directors of PWD, in the
    abovementioned position;

    WHEREAS, in the event of vacancy in the position of the member of the Board, an
    appointment by the appointing authority, in accordance with PD 198 (as amended by PD 768) shall
    take place.

    NOW THEREFORE, premises considered, on the motion made and duly seconded without
    objection, the Board of Directors jointly RESOLVED as it is hereby RESOLVED to appoint Mrs.
    Amelia M. Tamayao as the new General Manager of Peñablanca Water District.

    RESOLVED FINALLY, that this Resolution shall take effect immediately upon approval of the
    majority members of the Board Of Trustees. Let the copy of this Resolution be furnished to the
    concerned officers and all appropriate government agencies for their information and guidance.

    UNANIMOUSLY APPROVED, this ___ day of ____ 2020 at Peñablanca, Cagayan.
