    EMPLOYMENT OFFER LETTER

    Dear Mr. Villa,

    Care 4 Life Trading (the “Company”) is pleased to offer you probationary employment
    for a period of six months on the following terms:

    1. Position. You will serve in a full-time capacity as __________________ of the


    Company. You will report to _________________________.
    2. Your primary duties will be:

    3. By signing this letter agreement, you represent and warrant to the company
    that you are under no contractual commitments inconsistent with your obligations to
    the Company.
    4. You shall be regularized after the six-month probationary employment period if
    you consistently meet the working standards of the Company as outlined to you in the
    course of our pre-employment interviews as well as during the company trainings. In
    order to qualify for regularization, you must observe all Company rules, policies, and
    regulations, pertaining to, but not limited to, punctuality, time-in, reporting, and
    attendance.
    5. Your performance will be regularly reviewed during the entire probationary
    employment period. The COMPANY reserves its right to conduct a review of your
    performance at any time, whether on a daily, weekly or monthly basis. The
    COMPANY, during such probationary period, shall have the right to terminate your
    probationary employment if you fail to meet the performance standards herein set out
    or meet company rules, policies, and regulations.
    6. You will be paid a salary at the annual rate of P_______, payable in semi-
    monthly installments in accordance with the Company’s standard payroll practices for
    salaried employees. This salary will be subject to adjustment pursuant to the
    Company’s employee compensation policies in effect from time to time.
    7. Your employment with the Company will begin as soon as you report for work
    and sign a Probationary Employment Contract.

    We hope that you find the foregoing terms acceptable. You may indicate your
    agreement with these terms and accept this offer by signing and dating both the
    enclosed duplicate original of this letter and returning them to me.

    This offer, if not accepted, will expire at the close of business on February 20, 2017.
