    having been assigned at Leyte Police Provincial Office, Provincial Drug Enforcement
    Unit, after having been duly sworn oath in accordance with law, do hereby depose and
    state:

    That I am not an applicant for UN Mission nor Due for UN Deployment;

    That I am not an applicant for Lateral Entry within the span of my IBC Schooling;

    That I have no pending administrative nor civil case filed against me in any court;

    That after completing the course, I will stay in Police Intelligence Unit for at least
    two years;

    That all the documents I am going to submit/upload in the online


    application/registration for the Intelligence Basic Course are true copy made from the
    original documents and;

    That I attest to the truthfulness of this undertaking and submit to the legal and
    administrative consequences thereof if ever the statements above are wanting in the
    truth and substance.
