Polio Campaign
1. Pre-campaign activity.
Develop and validate POLIO Microplan Attend UCMO trainings and also attend team trainings Also
validate the field.
Make registration of High risk population.
Update school registration also update the registration of under five year children in this School.
2. During Campaign.
Take house hold cluster, find out teams issues, Inform UCMO and PEOs about team issues and solve the
team’s problems.
3. Post campaign.
Take house hold clusters, Make PCM cluster, Take survey of market and find out the missed children also
inform to UCMO and PEOs.
AFP surveillance:
 I collect the zero reports of every week regularly and submit in office.
 Meet to Formal / informal healthcare provider also asked them any AFP or POLIO case are notified by them
and also I inform about case to PEOs.
 If any AFP case found in my union council I also start the stool sample process and collect stool to WHO
office. If any zero dose AFP case found then I take 30 House cluster in send it to PEOs.
