    Petitioners to this Honorable Office most respectfully states that;

    1. Petitioners are Filipinos of legal ages, and with residence at Brgy. Zone IV and Rizal
    Sogod Southern Leyte.

    2. Petitioners are the spouse and daughter of ILDEFONSO CATIGAN who is a grantee of
    an ORIGINAL CERTIFICATE OF TITLE from the Department of Environment and
    Natural Resources (DENR) with KATIBAYAN NG ORIGINAL NA TITULO BLG. 51592,
    KALOOB NA PATENTE BLG. 086417-05-21710.

    3. The said Title there appears a lien/cadastral lien or annotation stating that No
    Transaction shall be registered unless the Cadastral Cost and Application fee will be
    fully paid.

    4. On May 21,2020 said cadastral Costs and Application Fees have already been paid.
    Attached herewith are copies of the Official Receipts duly issued by the Department
    of Environment and Natural Resources (DENR) as proof of payment thereto.

    We, FLORENTINA VITAL CATIGAN AND EDWINA CATIGAN TELIN, of legal ages Filipino and
    residents of Brgy. Zone IV and Rizal, Sogod Southern Leyte after having been duly sworn in
    accordance with law dispose and states that;

    1. We are the petitioners (spouse and daughter) of the owner of the real properties
    mentioned herein.

    2. We have caused the preparation of the same and that we have read the contents
    thereof.

    3. We hereby certify that all allegations therein are true and correct to our personal
    knowledge.
