    In the exigency of the service, you are hereby designated as SCHOOL 4Ps COORDINATOR of
    BRAULIO G. DANO ELEMENTARY SCHOOL, Makilala Central District, effective immediately.
    This designation is effective until such time it is revoked by this office.
    With such designation, you are expected to perform the following official duties and functions
    inherent to the position: (D.O. 99, s.2010)
    1. determines the 4Ps beneficiaries by accepting the DSWD School Enrolment Certificate Forms of
    beneficiaries for verification and signing;
    2. monitors the beneficiaries’ monthly attendance and school progress through proper coordination with
    class advisers;
    3. coordinates with the School Health Coordinator regarding the nutritional status and availment of
    feeding and deworming programs of beneficiaries;
    4. coordinates with parents and DSWD personnel regarding school-related concerns of beneficiaries;
    5. maintains files/reports of the beneficiaries for reference;
    This assignment/designation is in addition to your regular functions as Teacher 1 and will not
    provide additional compensation/salary nor claims for priority in promotion, except reimbursement of
    actual travelling expenses while on official travel.
