Gentlemen:

This has reference to your letter dated ______________ requesting reinvestigation/


reconsideration on certain items included in our letter of demand dated __________ and
Assessment Notice No. ______________ covering taxable year/period __________. In order for
this Office to act on your request, you are hereby advised to immediately settle the deficiency
tax(es) attributable to the undisputed findings arising from the investigation of your tax return,
computed as follows:

In view hereof, you are requested to pay the aforesaid deficiency tax(es) within the period
shown in the above computation to avoid the imposition of additional interest. Immediately upon
payment, please furnish this Office a copy of the duly validated Payment Form and other proof of
payment so that we can proceed with the evaluation of the disputed issues embodied in your letter
of protest. No action shall be undertaken on the protested issues proof of payment on the
undisputed portion of the assessment is received by this Office.
