A nurse’s responsibility includes the identification of concerns of patients upon
admission to any Neonatal Intensive Care units or during bedside rounds. Choose from among
the following newborn who are at high-risk by writing True if the statement is correct and False
if otherwise.
___x____ Baby girl Sweetheart appeared jaundice a day after birth the jaundice disappears at
her 7 th day.
RATIONALE: The baby is still premature, there is no enough RCB so Jaundice in newborn is not normal
especially for premature babies. And mostly they have condition of hyperbilirubemia wherein there is
too much bilirubin in your baby’s blood.
___x____ A baby was born with an AOG 37 weeks. With birth weight of 2.20 kg. Appeared to be
pinkish with regular breathing and CR of 140 bpm.
RATIONALE: High risk because AOG is 37. The weight is 2.20kg so it is not normal. The normal weight
is 2.5-2.3kg.
___/____ Baby Theresa who was 2 weeks old with soft and downy hair all around her trunk and
neck and appeared to be jaundice.
RATIONALE: A baby with soft and downy hair all is normal for newborn, this is called lanugo. And
trunk and neck appeared jaundice and this is normal for a newborn because this will disappear 1-2
weeks.
___x____ Baby Anna was born together with a dark green sticky amniotic fluid and her body is
coated with whitish cheezie substance.
RATIONALE:Amniotic fluid is not normal because it is green, so the normal appearance is clear. This
means that the baby inside ate his/her poop itself then it inhaled to its lungs. MAS (meconium
aspiration syndrome) is present.
___/____ Baby Rose was born with AOG of 42 weeks, pinkish in color , loud cry and active
movement with a birth weight of 3.5 kg.
RATIONALE: 38-42 is normal AOG. This is not a post mature since 42 weeks onwards are the factors of
it. Pinkish in color, loud cry and active movement with is very good condition for a newborn. And the
birth weight of 3.5kg is normal for a newborn.
