OFC Quality Issues

1. Splice Loss value not matching with SOP and RFP due to splicing of different
make ADSS Cables (HTL and Sterlite). High Loss > 0.1 dB observed.
2. Labelling and Tagging not done for incoming and outgoing cables at street box
locations. Mandatory for cables / tubes, splitters and FDMS modules as per SOP
(KFON/QA/QC/GPON-03 REV-1.0)
3. 4F Cable Termination splicing inside FTB Type-1 done for maximum End
Offices before installation of 9U Rack. Due to this, not feasible to channel the
Drop Cable inside 9U Rack through cable inlets provided at top of rack.
9U Rack must be mounted first in the officially allocated space as per SOP
(SAT End Office version 1.2)
4. Pole Stay Wire not installed for highly inclined KFON Poles and not installed even
for accident prone area.
5. Cable Tagging for Loops inside ACSB is mandatory because in the long run, most
loops are falling down.
6. Cable Loops not maintained as per SOP (some places less, some places more).
7. Cable Restoration is not proper, taking excessive time to restore the fallen cable
which causes more damage for cable.
8. For UG work, not maintaining the minimum depth for OFC duct installation as per
SOP. No guidelines mentioned in the SOP for UG cases where depth of trench is
less than 600 mm for rocky terrain.

9. Manhole installation mandatory at entry and exit pit location in case of UG works
in order to maintain the minimum bend radius.

10. Not clearing given physical punch points in required time period leads to revisit in
the field, due to that AT process gets further delayed, rectification works are
going in a dead slow manner.
11. Critical issue like tagging on Jio Poles not resolved yet.

12. Type-2 Closures are used for Midspan instead of Type-1 (Dome shaped). Must be
Type-1 Closure as per GPON Design Document (version 1.4.1)

13. GI Pipe must be considered for duct protection at Aerial to UG conversion areas.
GI protection is mandatory in case of flood prone area.
Thrissur: Chalakudy AEO, Vettukadavu Bridge
14. No trenching done from bridge end to nearest pole (15 Metres). No protection
given for duct. PCC done directly above duct despite PMA reminder.
Alappuzha: Mannar AEO, Mithramadam Bridge
15. Fiber Restorations pending in Kottayam and Kasaragod. In some areas, fiber has
fallen down across the road, which causes more damage to the fiber.
16. MH (Manhole) installation is not proper in substation premises and needs proper
rectification. Kottayam: Ramapuram AEO
17. Fiber laying near the transformer has quality issues. In some areas, OFC was laid
under the transformer and extra cable kept inside the transformer cage. This
major quality issue needs timely rectification.
18. For all the AEOs, ACSB height is not as per SOP and needs rectification for the
same. Black cable ties need to be use for proper arrangement of the cable.
19. Excess Sag in ADSS cable noticed at many locations, need rectification at the
time of FRT Activity.
20. Link needs adequate quality inspection and rectification. There are major quality
issues in fiber installation. Kottayam: Pala AEO
21. In order to maintain the minimum height along sides of the road plus road
crossing locations, all members of the laying team must be aware of the ADSS
laying guidelines mentioned in the SOP.
22. 4F Drop Cable not routed neatly inside many End Offices and cable not
channeled through rack inlet.
23. Proper Holes must be provided for cable entry from Roxtec of POP to FDMS
(Cable Tray to FDMS) in order to facilitate maintenance related activities.
24. Installation of Equipment inside POP should be as per the equipment layout.
25. Punch Points for all links must be properly rectified before offering for AT to save
time and avoid repeat visit of sites.
