6) BRIEF RESUME OF THE INTENDED WORK

6.1) Need for the Study:

Any opacity in the lens or its capsule, whether developmental or acquired is called as Cataract. 1 Age
related cataracts are responsible for 51% of world blindness, about 21million people 2 and the condition is almost
universal in varying degree in persons over 40 years of age. It occurs equally in men and women.

Ayurveda has explained Timira roga under drishtirogas.3 Clinical features of dwitheeya patalagatha timira
such as vihwala drushti, dooram sukshmam cha na eekshathe etc. can be correlated to that of immature cataract.

In conventional system of medicine, the only definite management is lens extraction once it reaches matured
stage or complete opacification. Over 90% cataract surgeries are successful 4 but complications though rare are
more serious which includes sensitivity to anaesthesia, uveitis, retinal detachment, subluxation of lens etc.5

The study of immature cataract bears importance because it can be reversible in the stage of lamellar
separation and prophylactic measures in the other stages of immature cataract can delay and prevent leading to
complete opacification.6 Medicines and procedures administered should be cost effective and easily applicable,
without any adverse reactions. Nasya7 and Anjana karma8 are described among the procedures mentioned in
Timira roga. Hence the present study is being taken up to evaluate the efficacy of Bhringarajadya taila Nasya 9
and Kumarika varti Anjana10 which can effectively arrest the progression of lens opacity, as per Ayurvedic texts.

6.2) Review of literature:



Dristigata rogas are explained depending upon the vitiation of doshas in various patalas. According to
Sushruta, vitiation of doshas in prathama, dwitheeya and tritheeya patalas is described as Timira 11; where
as Vagbhata mentions that Timira is occurring in prathama and dwitheeya patalas.12

6.3) Objectives of the Study


 To evaluate the efficacy of Bhringarajadya taila Nasya in the management of Timira.
 To evaluate the efficacy of Kumarika varti Anjana in the management of Timira.
 To compare the efficacy of Bhringarajadya taila Nasya and Kumarika Varti Anjana in Timira.

7) MATERIALS AND METHODS:


7.1) Source of data
The patients with clinical features of Timira will be selected from OPD and IPD of Shalakya Tantra
Department of Sri Kalabyraveshwara Swamy Ayurvedic Medical College, Hospital and Research Centre,
Bangalore.

7.2) Method of collection of data


 It is a comparative clinical study, where in 40 patients of either sex, diagnosed as Timira, will be
randomly assigned into 2groups, each comprising of 20 patients.
 A case proforma will be specially designed with all points of history-taking, physical signs and
symptoms.
 The parameters of signs and symptoms will be scored as mentioned in the proforma.
 The data obtained in both groups will be recorded, tabulated and statistically analysed using suitable
statistical methods.

Diagnostic Criteria
 Patients presenting with clinical features of Timira.
 Diminished Visual acuity up to 6/36
 Immature cataract confirmed by ophthalmoscopy, slit lamp biomicroscopy and presence of iris shadow.
Inclusion Criteria
 Patients with signs and symptoms of Timira.
 Patients fulfilling diagnostic criteria.
 Age above 40 years.
 Disturbance in vision (diplopia, polyopia, halos)

Exclusion Criteria
 Post surgical Cataract.
 Mature Cataract
 Sluggish pupil reaction
 Complicated Cataract ie.cataract secondary to other ocular diseases like Glaucoma,
Optic atrophy, Macular degeneration, Posterior segment disorders.
 Nasya anarhas
 Anjana anarhas

Study design
 This is a comparative clinical study with pre test and post test design will be conducted on 40patients
with signs and symptoms of Timira or Immature cataract.

Intervention
40 patients of Timira will be selected randomly and divided into 2 groups, Group-A and Group-B.
Group Chikitsa Vidhi Avadhi
A Nasya with Sthanika 3 Sittings of 7days Nasya karma
Bhringarajadya taila abhyanga and with an interval of 9days- total
Bashpa sweda 48days
followed by
Nasya- Once
daily in the
morning, 8drops
in each nostril.

B Anjana with Once daily in 48 consecutive days.


Kumarika varti the morning

Duration
Total duration of treatment shall be 48days in each group.

Assessment Criteria:
Assesment of the clinical study will be done on subjective and objective parameters based on the proforma.
