    AFFIDAVIT OF LOSS

    I, JAYSON P. RICHARDS, of legal age, and resident of Brgy.


    San Lorenzo, Kawayan, Biliran, after having duly sworn, depose and
    say, that:

    1. I am the registered owner of a parcel of land located at


    Poblacion, Kawayan, Biliran, containing an area of 450 square
    meters, more or less, described and covered by Transfer Certificate
    of Title No. T-12345 of the Register of Deeds Biliran.

    2. I used to keep my Owner’s Copy of the said title in a plastic


    folder containing all my important documents at my aforesaid
    residence. On the 1st of June 2021, I discovered that the said Owner’s
    Copy was missing from the said folder. Thinking it was only
    misplaced, I exerted efforts to locate the document, but cannot locate
    it anywhere inside my residence. Thus, I considered the same to
    have been lost and beyond recovery.

    3. I did not sell, mortgage or in any manner dispose of my said


    Property to whosoever; and that there is no pending transaction
    involving said property for registration with said Register of Deeds.

    4. I have executed this affidavit to attest to the truth hereof; and


    for purpose of filing the necessary petition for issuance of second
    owner’s copy of TCT No. T-12345; and for legal intents and
    purposes.

    FURTHER, AFFIANT SAYETH NAUGHT.

    affixed his Thumbmark in the foregoing affidavit before me and in the
    presence of the two (2) witnesses whose signatures appear below
    this thumb signature, and who acknowledged before me that he
    executed the same voluntarily.

    The undersigned further certify that the contents of said affidavit


    was explained by me in (English or Filipino), a Language known and
    understood by the affiant, he being blind and appears to have
    understood the contents thereof.
