Dear Ms Kork,
Re: Ms Caly John, 80-years-old

I am writing to update information regarding the current health status of Ms Caly John who
requires continuing care and management at your facility. She is recovering from stroke and
she is being discharged today.

Ms John was admitted to hospital on 16 April, 2019 after a syncope episode, dizziness and
palpitation. Her ECG showed tach-brady syndrome. Therefore, pacemaker implantation was
done on 17 April, 2019. After two days, she had a stroke and a MRI revealed acute ischemia,
which was treated with thrombolysis. She has been commenced on Aspirin, 75 mg, with her
regular medication.

Presently, she has comprehensive improvement; however, her speech is slurred.

It would be greatly appreciated if you could arrange aid for activities of daily living, kindly
contact physiotherapist for continuing exercise. Please contact occupational therapist for home
rearrangement such as rail. It is important to inform speech and language therapist visit for
every week to check her eating and drinking regime. Please remember to add thickener in all
drinks until advised by speech and language therapist.
