PATIENT ASSIGNMENT

RATNESWOREE PRAJAPATI

Specific objectives:
Definition
 Assignment is delegation of certain duties to
personnel concerned in a particular situation.

 Assigning means making arrangements for


personnel to work. This is done by head nurse or
incharge through good planning.

 The method(s) by which the patient care


load is distributed among the nursing personnel
available to provide care.

Purposes:
 To provide patient with best possible nursing
care.
 To provide a well rounded education
experience for student nurses.
 Suggest appropriate procedures for nursing
care, with maximum efficiency with minimum
effort.
 To gain the cooperation of workers in the
acceptance of work to be done.

 To plan assignment, which are stimulating,


interesting to professional growth.
 To achieve good ward management.
 Serve as work plan.

Formulate assignment of activities based on the


following principles:
 It should be made by the head nurse.
 Planned weekly or from day to day
 The head nurse must know the nursing
ability of each nurse
 The head nurse must know the nursing
needs of each patient and time
 Use simple clear words.
 Fix responsibility for all duties clearly with
no overlapping.
 Arrange the various parts of the assignment
in logical order.
 Ensure continuity of patient care.
 Assignment should be made in accordance
with the administrative policies.
 Planning for skills development
 It should be in written form.
 Based on accurate assessment.
 It should be related to previous assignment.

 Assignments should not be changed very


offenly

 Assignment planning is closely related to


time planning.

 Assignments should be assigned to one


nurse is geographically close together.
Methods of assignment
• Functional Methods of Assignment
In this method, one staff is assigned to
one/two tasks and she has to perform the
same task for all patient in the ward.
Example: One R.N performs all assessments
and administers all intravenous medications;
and the assistant performs hygiene tasks and
takes vital signs. Incharge nurse makes the
assignment and coordinates the care.

Advantage

 Large number of tasks in short period of


time.

 Unskilled person can be trained to one


specific task.

 Cost effective to staff.


 Need less equipment
disadvantage
 Patient receives fragmental care.

 Poor communication between staff and


patient

 Professional nurse feels unsatisfied.

 Less control of quality.


2. Case study ( Total patient center)
 The case method or total patient care
method of nursing care delivery is the oldest
method of providing care to a patient.

 The premise of the case method is that one


nurse provides total care for one patient during
duty period.
Patient centered method
 In this method a nurse is assigned to give
complete nursing care to assign patients (one or
more)
 Total patients care including: General
nursing measures, Treatments, Medications,
Taking TPR, diet and giving emotional support
and education required on health.
 Patient care is supervised and evaluated by
the co-coordinator or head nurse.
 The patient has different nurse in each shift
and there is no guarantee of having the same
nurse each day.

Advantages
 The nurse can see better & attend to the total
needs of clients.
 Continuity of care can be facilitating with
care.
 The nurse and patient exchange mutual trust
and can work together towards specific goals.
 Client may feel more secure. (Patient
Satisfaction)

Advantages
 Educational needs of the clients can be
closely monitored.
 Work load for the unit can be equally
divided among the available staff.

 Nurse’s accountability for their function.

disadvantage
 Many clients do not require this type of
services.

 It may be expensive.

 It’s difficult for nurse to use this method to


become involved in long term planning and
evaluation of care.

 The greatest disadvantage of care nursing


occurs when the nurse is inadequately trained or
unprepared to provide total care to the patient.

3. Team nursing
 This is a new concept of assignment. The
team is guided by a professional nurse and they
work together to provide individualized patient
care.
 Regular conference is held by team leader to
discuss and plan the care with the members of
team.
 The team leader guides the team members in
understanding the needs of the patient who
plans interprets.

 A team leader is responsible for coordinating


a small group of licensed & unlicensed
personnel to provide patient care to small group
of patient.
 The team leader assigns each member a
patient or a specific responsibility.
 The member of the team report directly to
the team leader who then reports to the charge
nurse or unit manager.
 There are several teams per unit & patient
assignment are made by each team leader.
advantage
 It includes all health care personnel in the
group functioning & goals.

 Feeling of participation & belonging are


facilitated with team member .

 Workload can be balanced & shared.

 Better nursing care will be provided to the


individual patient.

 Division of labor allows member the


opportunity to develop leadership skills.

 Every team member has the opportunity to


learn from & teach colleagues.

 There is a variety in the daily assignment.

 Interest in client’s well being & care shared


by several people; reliability of decision is
increased.

disadvantage
 Establishing the team concept takes time,
effort & constancy of personnel.

 Unstable staffing pattern make team nursing


difficult.

 The team leader must have complex skills


and knowledge eg: communication decision
making, leadership skill, co-ordination.

DISADVANTAGE

 There is less individual responsibility and


independence regarding nursing function.

 The client is difficult to identify personnel


who are responsible for his care.
 All care is directed by a registered nurse if
teams are constant.

 Barriers between professional and non


professional workers can be minimized; the
group effort prevailed.
