NARRATIVE REPORT
ACTIVITY: Orientation on Annual Physical Examination for DepEd Teaching and
Non-Teaching Personnel

1. INTRODUCTION
As stipulated in Division Memorandum No. 374 s. 2021, Tanay North
National High School conducted an orientation regarding online health screening
of DepEd Rizal teaching and non-teaching personnel. This is spearheaded by our
Division Office Nurse, Ms. Brenda A. Inocencio.
2. AGENDA/OBJECTIVES
The objectives of this activity are as follow:
a. transfer information from resource person to disseminate the Division
Memo No. 374 s. 2021 entitled Online Health Screening of DepEd
Personnel;
b. address some of our queries regarding health related issues and concerns
and how physical examinations may affect our fitness to work;
c. determine the status of our health; and
d. have open communication with our doctor relative to our health and COVID
19 issues.
3. HIGHLIGHTS/FLOW OF PROGRAM
The first part of the program formally started at 2:10 on the afternoon with
the moderator, Mrs. Maria Paulina Rose N. Rubin. An audio-visual presentation of
national anthem was played followed by a prayer led by Mrs. Ma. Guadalupe G.
De Guzman, School Guidance Coordinator. Then, Mrs. Elenor A. Capistrano
checked the attendance of the participants. After this, Ms. Bernadette V. Tonito
shared a Food for Thought that highlighted the trust we shall have towards God in
times of uncertainty. This was then followed by welcome remarks by the OIC Mr.
Christian Ronel F. Austria in behalf of the school head Dr. Mary Jane T. Espiritu.
Next is the statement of purpose of the activity given by Dr. Mary Catherine O.
Flores, School Clinic Coordinator. Then, Ms. Mylene E. Don introduced the
resource speaker of the orientation proper.
Ms. Brenda A. Inocencio discussed the content of Division Memorandum No. 374
s. 2021 highlighting the SDO Rizal’s concern and recognition to the needs of the
teaching and non-teaching personnel to be healthy and resilient considering that
their physical well-being has a remarkable impact on the delivery of services
during this pandemic.
The online health screening was emphasized considering the safety
protocols implemented by IATF. This online health screening is in lieu to the F2F
annual physical and dental examinations to lessen the mobilizations of personnel
to ensure their safety.
According to her, the medical online health screening will start on
September 1, 2021 – November 30, 2021. Links to the online screening were
given. Requests for diagnostic health examinations will be given by the nurse
assigned to the sub-office. The laboratory results shall be submitted dated 3
months or less from the date of laboratory tests. She mentioned the different lab
tests for teaching and non-teaching personnel.
After the discussion, question and answer was steered. Teachers asked
their concerns and these were addressed by the speakers. The “fitness to work” is
gighlighted to be seen on the medical certificates to be uploaded not later than
Nov. 30, 2021. The resource speaker even gave her contact info for further
clarification and updates. Likewise, with the dentist of SDO Rizal.
Then, the closing program formally started with the awarding of certificates
to the resource speaker. The closing remarks was given by Mr. John Michael S.
Omido, MAPEH Coordinator and was followed by closing prayer by Mrs. Cheryl M.
Delda, TLE Teacher.
The orientation ended at 3:15PM with 100% attendance of the faculty
members.
4. RESULT
The key result of this activity was the information shared to the participants
regarding the annual physical and dental examination which will not be in F2F
mode but instead this should be through online screening. The important dates
were noted and key ideas were properly highlighted.
5. RECOMMENDATION
It is recommended that teachers must comply to the needs of online health
screening. They are given the chance to have their contact to the designated
nurses and dentists for the sub-office.
