Q. What is jsa? How to conduct jsa?


 It is technique to identify the hazards in each step of activities and to take the
control measures to eliminate the hazard or reduce the risk to an acceptable
level.

The basic steps to conduct JSA are:

 selecting the job to be analyzed


 breaking the job down into a sequence of steps
 identifying potential hazards
 determining preventive measures to overcome these hazards

Q. what you mean frequency rate and severity Rate?

 Frequency Rate: - It is a method to calculate the average recordable injury rate in a year with
the formula -total recordable injury x 200000 divided by total man hours worked
 Severity Rate: - The severity rate is the calculation that gives a company an average of number
of lost days per recordable incident.

Q. List out the safety precautions can be advised in safe handling and
storage of compressed gas cylinders?
 Handling of compressed gas cylinder:- Make sure the cylinder is fitted with correct type of
regulator, Never use grease or oil to lubricate cylinder regulator or cylinder valve, Correct type
of fire extinguisher must be available when the cylinder is in use.
 Storage of compresses gas cylinder:- Gas cylinder must be kept in trolley and secured at all
the time, Protective caps shall be secure when not in use, Compressed gas cylinder always
stored in upright position, Oxygen cylinder must be stored separately from the flammable gas
cylinder and empty cylinder must be stored separately from the filled cylinder.

Q. Describe the different types of fire extinguisher and their application?


What are the elements in fire?
 Fire extinguisher has been classified into four class.
 Class A fire, fire involve in wood, paper, plastic, thrash etc. (water type extinguisher)
 Class B fire, fire involve in flammable liquid or gas. (Foam type extinguisher)
 Class C fire, fire involve in electrical equipment or panel. (DCP or CO2 type extinguisher)
 Class D fire, fire involve in metal eg. Sodium, Magnesium, Lithium. ( special metal type fire
extinguisher)

Q. Define Heat Stress? Write down the symptoms and control measure.

 Heat stress is a most serious disorder, it occur when the body temperature regulation fails and
the body temperature rises to a critical level.
 Symptoms of Heat Stress:- a- Dizziness and light headache, b- Lack of sweating despite of
heat, c- Red, hot and dry skin, d- Muscle weakness and cramp, e- Nausea and vomiting.
 Control measure:- Reduce the time of work under direct sunlight, Drink plenty of cool
water, Provide cool shaded work area, Provide air-conditioned rest area, Increase the
frequency and length of rest break, schedule job to the cooler time of the day, Caution worker
to the direct sunlight, Trained First Aid provider shall be available to give treatment in the
event of heat related illness.

Q. What are the hazards created by static electricity? How can you control
or eliminate static electricity?
 The main hazards of static electricity are fire and explosion and electric shock. To control static
electricity we need to ground all the electrical equipment and during refilling follow grounding
and bonding system, reduce the flow rate of liquid during filling.

Q. What is work permit/ how many types of work permit in sabic?

Work permit is a written legal document which authorized a person/group


of person to execute a job on a specific task on mention equipment for
given time duration.

1. Hot work
2. Cold work
3. Confined space
4. Electrical permit
5. Radiography

Q. What is pipeline / what is the purpose of pipeline?

it is media to transport oil, gas & water from one location to another location

Material Safety Data Sheet is technical document which provided detailed and
comprehensive information related on a controlled product.

Q. what is confined space/what should be oxygen leave in confined space?

Any enclosed that has limited entry and exit and design for continuous human
occupancy. oxygen level in confined space 19.5 to 23.5 %

Q. What is hazard?

Any thing that has the potential to cause harm to human and damage to property
and environment.

1. to collect the information, you need to pass on the enforcing authority


2. to identify the cause of the accident and /or incident to stop it happening
again
3. To get information needed for insurance claim
