TABLE OF SPECIFICATION

Procedures:
The Table of Specification (TOS) is accomplished in the formulation of the test questionnaire for midterm and final examinations. The following are the procedure:

1) The teacher secures the TOS form from the Office of the Dean or at the Program Chairperson’s Office.
2) The teacher submits the accomplished TOS incorporating the prescribed percentages (with the questionnaire) to the Subject-Area Coordinator (SAC) ten days before
the scheduled exam.
3) The SAC checks and approves the TOS and questionnaire. TOS and questionnaire which need improvement are returned to the teacher and these will be
resubmitted to the SAC for approval.
4) Approved TOS and questionnaire are submitted by the SAC to the VPAA Office c/o Office Assistant or Academic Records Assistant for logging-in and for printing at
the Printshop.
