Survey Questionnaire for the Marketing Plan of Hubble Bubble Tea House

We are 2nd year Tourism Management students of the School of Hospitality and

Tourism Management of Holy Angel University presently enrolled in Tourism and

Hospitality Marketing. In partial fulfillment of the said course, we are tasked to come

up with Marketing Plan for Hubble Bubble Tea House, an online food business. Rest

assured that whatever information gathered will be treated confidential.

Informed Consent

I have been invited by Vince Gill M. Manganti, Nea Ranee P. Marquez, Jennika A.

Mendoza, Diana A. Mercado, and Carl Albert M. Miranda, researchers from Holy

Angel University, who are currently collecting data needed for their marketing plan

on Hubble Bubble Tea House located at San Roque, Bamban, Tarlac.

We are 2nd year students taking up BS Hospitality and Tourism Management at

Holy Angel University. The purpose of the marketing plan is to know the Customers’

and the Potential Customer’s Satisfaction towards Hubble Bubble Tea House and to

determine the results of the factors affecting the business. Likewise, this research

will identify the strengths, weaknesses, opportunities and threats of the business

establishment to draw some recommendations that will help the management. This

paper will be presented and submitted as part of our requirement for the subject

Tourism and Hospitality Marketing (TOURMARK).

I will be asked to answer an online survey in Google Forms, which may take 10-15

minutes. Once the maximum number of eligible respondents is achieved, the survey

shall be taken down, and responses shall be exported in an MS Excel file to warrant

data analysis. All information shall be treated with utmost confidentiality and will be

solely used for the purpose of this marketing plan. Reporting of the results will be

done collectively. Individual results will not be handed to the administrator/institution

that I am currently affiliated with, hence will not in any way affect my relationship with

them. I was informed that there is no known risk in participating in the study and no

direct benefits or monetary compensation shall also be handed. My participation in

this study is voluntary and it implies that answering the questions is out of my free

will. I will answer the online survey as honestly, accurately and completely as I can

for the marketing plan to serve its purpose. I am also aware that I can withdraw

anytime from the study although I have already given my consent during the first part

of the survey.
