TEST I. ESSAY. (40pts)

1) Differentiate an initial investigation, a progress report and a final report. (10pts)

Initial investigation - means those activities conducted by Department investigative staff to


determine whether a report of suspected child abuse or neglect is a good faith indication of
abuse or neglect and, therefore, requires a formal investigation. Investigation Report – is
considered internal communication. Investigation report iscomplete when it answers the six
interrogatives of police report writing- the5W’s and 1H.It also provides in brief, the sworn
statements executed by the individuals having an interest in the case being investigated.
How To Write An Investigation Report
1. Start With What You Know. The investigator should record the information about the subject
(respondent or accused person) and the details of the complaint.
2. Make Sense of What You Have. Evidence analysis can substantiate claims from either party.
3. Formulate a Conclusion.

2) Progress Report – it is an accounting of the actions or series of actions undertaken in

relation to an ongoing investigation of a case.

3) Final Report – Is a thorough, in-depth and lengthy account regarding an investigation

into an incident or case as mandated by higher authorities to establish a determination of

the truth and or how far it could be determined based on the facts and circumstances

with the appropriate recommendation for the proper course/s of action/s to be made.

2) Give the similarities and/or differences of a spot report and an incident report. (10pts)

Spot Report refers to an immediate initial investigative or incident report

addressed to Higher Headquarters pertaining to the commission of the crime, occurrence

of natural or man-made disaster or unusual incidents involving loss of lives and damage

of properties. Similat to Spot Report an Incident Report is a victims statement regarding a crime often
bank statements, letters and other supporting documents are included. These reports are generally not
investigated and are for insurance purposes requesting police report.
3) Give the importance of investigator’s notes in doing his report. (10pts)

Complete, accurate and properly maintained notes are a fundamental aid to memory and serve
as a guide during investigations and interviewing witnesses, victims, suspects and accused
persons

4) What is Flash alarm? Give its importance. (10pts).

Flash alarm is an account relative to the commission of a crime incident filed by complaining witness of
a victim to the police citing the date, time, place possible motive of the commission/ occurrence, how it
was perpetrated and other details relative to incident. It is important because it provides a detail
information that can be used as strong source of data about the reported incident, it is also important
for documentation and for an immediate evaluation or action on the case.

TEST II. Scenario (15pts each)

Instruction:

1. Make a spot report based on the scenario given.

2. Make a progress report on the scenario given.

3. Anticipate the scenario given and make your Final Report.

4. Make an investigation report.

Scenario:

PSMS Simeon Laban has been sleepless for two days. He, along with fellow operatives, has

been trailing a group of suspected carnappers believed to be responsible for a string of carnapping

cases in Metro Manila. Acting on a tip, he follows the suspected leader of the group who embarks

on another carnapping attempt. On board a 2009 flaming red Toyota Vios, the leader, along with

two cohorts, blocks a 2011 white Hyundai Tucson with plate number UVC 789 driven by a middle

aged woman. The leader alights, takes his cal. 45 pistol tucked from his waisthand and points the

same to the woman driver of the vehicle. PSMS Laban, who is on board a kia car with plate number

OVP-221, immediately gets off the trailing vehicle, and with two other operatives, calls the attention

of the carnappers who are caught unawares of the police presence.


Jolted and apparently confused, the carnappers rush back to their getaway car in an attempt

to escape from the police. They speed away, with the police in hot-pursuit. Sensing danger, the

armed carnappers fire at their pursuers—missing their target. The policemen then engage the

suspected criminals in a running gunbattle. Minutes, later the suspected leader of the group lies

dead while his two companions raise their hands in surrender – shaken but unscathed. Police

reinforcement arrives and nabs the surviving carnappers. Later, soco cordon off the area for

processing the crime scene.

1. On November 8, 2021 at around 3:08 pm car napping incident transpired at Metro Manila
wherein the victim was identify as Nelmida Y Cruz, 46 years old, married, Teacher, and a
resident of Brgy.Libis, Quezon City, while the three 3 unidentified suspect with 2009 flaming
red Toyota Vios try to stole and carted away one (1) 2011 White Hyundai Tucson bearing plate
no. UVC 789. PSMS Simeon Laban along with fellow operatives has been trailing a group of
suspected car nappers. At the moment the suspect perform their illegal activity PSMS Laban,
who is on board a kia car with plate number OVP-221, then engage the suspected criminals in
a running gun battle. Minutes, later the suspected leader of the group lies dead while his two
companions raise their hands in surrender – shaken but unscathed.

2. This pertain to carnaping incident that transpired Metro Manila at around 3:08 pm of
November 8 2021 Be inform that the suspect Cardo Batumbakal 34 years old, married, Laborer
and a resident of Brgy. Rosario, Pasig City was shot by PSMS Simeon Laban. Said suspect was
brought to Mission Hospital for medical treatment but was pronounced dead on arrival.
3. The cadaver of the victim was move to PNP Crime Laboratory Office, Camp BGen Rafael T
Crame, Quezon City for autopsy examination. The immediate cause of hist death qas gunshot
wound on the neck, head and trunk.
4. Further development will be sent to that office as soon as obtained
