Technical Comments: Flow limitation was observed, Witnessed Apneas.

PROCEDURE:
Single overnight attended comprehensive diagnostic & therapeutic sleep study (level I/II) done using a portable
data acquisition device.

REPORT:
The baseline sleep architecture was fragmented due to frequent arousals and wake periods. The sleep latency was
2.0 minutes. The sleep onset to REM sleep latency was 208.5 min minutes. Total sleep time was 404.5 minutes
with a sleep efficiency of 95.2% The REM sleep accounted for 28.3% and slow wave sleep was 15.6% of the total
sleep time.

The AHI was 62.3/h in the diagnostic part of the study. The lowest oxygen saturation recorded was 65% from the
baseline of 95% while awake and breathing room air. Most of the events were obstructive and were associated with
severe oxygen de-saturation. Snoring was severe throughout the diagnostic part.

TITRATION: Manual Titration Started (AUTO CPAP) with (Nasal Mask) in the latter half of the night and the
AHI dropped to 10.5/h. The lowest oxygen saturation recorded was 90% from the baseline of 95% while awake
and breathing room air.

PLM: The leg movements were not significant.


ECG: Patient was in regular sinus rhythm throughout the night without significant arrhythmias.

CONCLUSION:
Sleep Disordered Breathing (SDB) with (SEVERE) Obstructive and Central Sleep Apnea leading to (Severe)
Oxygen desaturation 65% and sleep fragmentation with good response to PAP Therapy with AUTO CPAP at 19
cmH2O.

RECOMMENDATION:

∙ Regular use of Auto Bilevel therapy device with Humidifier during sleep.
∙ Lifestyle modification and good sleep hygiene practices.
∙ Avoid supine position sleep.

Study Indication:

Polysomnography was conducted on the night of 28-12-2021. The following were monitored: central and occipital EEG,
electrooculogram (EOG), submentalis EMG, nasal and oral airflow, thoracic and abdominal wall motion, anterior tibialis EMG, body
position and electrocardiogram. Arterial oxygen saturation was monitored with a pulse oximeter. The tracing was scored using 30
second epochs. Hypopneas were scored per AASM definition.
