1. PURPOSE : To encourage the people to report the unsafe act & condition and taking corrective & preventive action and thereby ensure the safe condition work place & create safety culture.
2. SCOPE : This procedure is applicable for Company & contractor employees of
Savli premises.
DEFINITIONS / ABBREVIATION : NM : Near Miss UA : Unsafe Act UC : Unsafe Condition CAPA : Corrective Action & Preventive action.
3. RESPONSIBILITIES : a) All the employees ( Company & Contractors ) areas in B&H and Heating is responsible for adherence of this procedure. b) Other responsibilities are as per description
4. DESCRIPTION :
RESPONSIBIL REFERANCE SR.NO. ACTIVITY ITY DUCUMENT Any company/contract employee on observing unsafe act/unsafe condition, NM incidence will fill the All people Near miss 5.1 NM form with CAPA & send it to concerned reporting near Format. department and copy to HSE department either in miss. Soft Or Hard form Near miss Record HSE engineer to register the NM in near miss register Register with 5.2 HSE & assign the Serial No to the NM. close out action & CAPA. Hazard HSE engineer to identify the Class A, B, C hazards 5.3 HSE classification category on the near miss. sheet 5.4 HSE Engineer to follow up for the compliance. HSE By Mail On completion of near miss’s compliance the concerned person will send the compliance report to Compliance 5.5 person who reported NM & copy to HSE department Department for the close out of the same. Near Miss HSE department will make summery of Near miss on Summery sheet 5.6 HSE weekly basis & department wise.
The summery report will be discussed in the Weekly
5.7 HSE supervisory meeting for further course of action.
