1. INTRODUCTION

The Duqm Refinery and Petrochemicals Industries Co (Duqm Refinery) Project is a


grassroots Crude Oil Refinery at Duqm on the Arabian Sea coast of Oman, approximately
600km south of Muscat.

The refinery is to process a range of Middle Eastern crude oils at a design rate of 230,000
BPSD.

The Refinery will be supplied with crude oil via a pipeline from the Ras Markaz Crude Tank
Farm (hereafter referred to as Sub-Package C). Crude will be transferred via pipeline to
Duqm Refinery (hereafter referred to as Sub-Package B) Refined products will be pumped to
the Port of Duqm where they will be stored and then loaded onto ships through the Terminal
Export facilities (here after referred to as Sub-Package A). The solid products of pet-coke
and sulphur will be transported by trucks to the Terminal Export Facilities (Sub- package A).

2. PURPOSE

The Purpose of this method statement is to highlight the general measures and precautions
required during activity of Erection of sulphur storage shelter. This method statement also
covers the method of site preparation involving mechanical execution like ground
assembling, Erection, Crane preparation etc. This method Statement is to ensure that all
information, materials, tools and equipment, manpower and supervision are available for the
execution of work. In particular, that work steps are and all safety measure are taken to
ensure that the work can be executed in a safe manner as per project specification.

3. SCOPE

Galfar Engineering & Contracting SAOG has been appointed as a SUBCONTRACTOR by


EPC CONTRACTOR to carry out the activities which include:

 ERECTION OF PETCOKE STORAGE SHELTER.

This method statement applies for carrying out the Steel structure erection within the
above scope of works.

8. RESPONSIBILITY
SITE MANAGER:

 Over all execution and administration of the project as per the contract requirements
Specifications and PQP.
 Shall be responsible for arranging resources for the timely completion of the Project.
 Execution of work as per the approved method statement
 Execution of work as per the Contract specifications, Approved construction schedule
and milestone targets.

QA/QC Manager:

Surveyor

 Ensure the foundation & concrete structures are as per drawing. Responsible for
checking the coordinates civil structure etc., provided by the Contractor.

structure for Contractor approval.

QA/QC Inspector:

 Coordination of all inspections and test requirement as per the specification requirement.

 Ensure through the independent lab technician that all lab tests and field tests are
conducted as per the specified frequency.
 Coordinate with the external laboratory for testing and timely submission of results.

foundations under services, if any, and all operation shall be conducted so as to prevent

any unnecessary destruction, scarring, or defacing of natural surroundings in the vicinity


of the work.
 Safety precautions shall be of paramount importance to prevent any danger to the public
and to the workforce and any operations carried out shall be closely monitored by the
Project HSE Advisors /HSE Inspectors.
 Before to start with the excavation activities in the area, it is mandatory to be sure that
the excavation to be done will not impact with underground facilities (power, telecomm,
pipe water, hydrocarbons, FOC, etc.).
 The site survey, before to sign the PTW, is mandatory. During this survey all the type of
check must be done in order to guaranty the safety of the activities to be done and in
particular the total respect, without any deviation, of the Golden rules.
 All the equipment shall be inspected by the safety department of subcontractor and
contractor and get the fitness certificate before its use to the site.
 The operator / Driver should have proper and valid required paper/document/licenses for
the operation of the vehicle / equipment.
 During the operation of the above equipment at site, for safe operation, flagman, banks
man shall be available. They shall be disposed of at every critical point and in any case
show us to prevent and avoid any possible incident due to noncompliance and or safety
distance from the equipment in operation.
 People working in the site must avoid to pass behind the operating equipment.
 All necessary permits shall be obtained as per Project’s safety regulations.
 All equipment operators are well trained, certified and licensed to operate in Oman and in
the Project.
 Risk assessment shall be constantly monitored for changes in conditions. Prior to
commencement of work every day the supervisor shall check all equipment against the
safety check list.
 A dedicated traffic management plan shall be defined in order to avoid any problem and
to be in condition to assess better the risks
 All machinery shall be periodically inspected and shall be kept in accordance with the
contract and HSE requirements.
 The day and night shift activities shall be carried out in accordance with the requirements
as described in HSE management plan.
 All excavation work to a depth greater than 1.2m shall be properly shored, sheet piled or
battered and shall be considered as a confined space.
 Housekeeping shall be maintained all times at site.
 Combustion emissions shall be minimized.
 All vehicle engines shall be switched off when not in use.
 Damage to land from the operation of Plant vehicles is to be avoided or minimized.
 There shall be no discharge of chemicals to land

 Spill & leaks of hydrocarbons shall be reported immediately to the HSE Advisor and
cleaned up in accordance with the relevant procedure.
 Spill kits will be carried in all vehicles and plant.
 Movement of the equipment shall be monitored by the banksmen, shift in charge and the
HSE personnel.
 All vehicle movements shall be regulated through designated route, where and if
necessary, in the critical points/crosses, the flag men will be used.
 Portable toilets fitted with WC are readily available in site for toiletry purpose.
 Cool water will be available for workers at all times
 All plant and vehicles will be checked before the commencing of each day’s work for any
leaks or visible defects.
 Strictly follow HSE guidelines provided as per the approved HSE management Plan.
 Tool Box Talk shall be conducted before commencement of work.
 All the equipment deployed at site to be approved by the Contractor.
 Erect sufficient warning signs and warning tape to prevent unauthorized access.
 Site access and egress have specified by the erecting sign boards.
 However, the site is assumed as green field, the underground utility services have been
traced and properly ascertained with the help of the drawings that received from the
Contractor.

Adverse weather conditions

 The emergency response plan will be alerted in case of adverse situation like rain, storm,
and hot weather. The speed limit of vehicle movement within the operational area of the

10.CONSTRUCTION METHODOLOGY and PROCEDURE

10.1 Equipment and it’s usage: -

 All the Erection equipment or Machinery shall be validated and copy of the Validation
certificate shall be maintained with each Equipment or Machinery.
 Calibration Certificates of the erection equipment’s shall be submitted with inspection
forms, for getting the approval from the Contractor.
 Erection work will be carried out by employing the services of a competent technicians,
subject to Contractor's prior approval.

 To provide adequate safe access / egress for supporting activities, vehicles and
pedestrian movement.
 To make firm, properly graded, drained area for safe storage of materials and the safe
operation of the equipment.
 Not to leave any loose steel parts, materials and / or tools irrespectively before leaving
the worksite.
 To barricade the erection areas adequately with warning signs to keep non-involved
people away from the area.
 Maintain specific required documents like PTW, JSA, Specific Erection Plan with crew
leader such as the relevant erection engineers and fabricator.
 The benchmarks only control & signalling the erection equipment(crane). Crew leaders
control team members for productivity and safe activity as per site engineer instructions.
 Contractor/Company approval shall be obtained for erection works/records, if requested

10.2 General information before Erection: -

 Protection of others, whether public or other workers, shall be safeguarded at all times.
Safety signs and barricades shall be posted where necessary to keep people out of danger
areas.
 A regular weather forecast should be obtained by the contractor in charge of the erection
program. Erection should not take place where weather conditions impose an adverse
effect, such as:
 High winds
 Rain or dew
 Poor visibility (fog, mist or glare)
 During windy conditions, the contractor and HSE department shall evaluate the relative
risk of all work performed when gusts reach 25 miles per hour.
 If erection work is stopped, measures should be taken to ensure that the
structure remains stable. After a stoppage, due to the weather, stability of the structure
should be checked before work is allowed to re-start.
 Employees who may indicate a fear of heights or are subject to dizziness shall be kept
on groundwork Employees are not permitted to ride loads or crane headache ball.
 Employees are required to wear a safety harness and the lifeline must be tied to an
object engineered to 5,000 pounds.
 Employees working more than 1.8 meters’ height shall be provided with fall
protection100% of the time.
 Projecting or protruding reinforcing rods which create tripping or falling hazards shall be
bent or covered as outlined in the Project standards.
 Excess material should not be hoisted to a structure and stored in the working area until
it is ready to be put into position and fastened.
 Before cutting any large or heavy structural steel member, the member shall be secured
or supported by wire ropes, cables, or other means to prevent dropping or uncontrolled
swinging.

 Keep working area in orderly condition with necessary equipment and materials safely
arranged.
 In setting steel, each piece shall be securely bolted before the load line is unhooked.
 The use of one-bolt connections is not appropriate.
 After full bolted while scaffolding activities life line shall be installed upon each beam at
every direction.
 When setting steel trusses, they shall be securely tied or cross-braced until permanent
braces are in place.
 Check loads to make sure there are no sharp edges, which will cut into lifting slings. Use
softeners where applicable.
 A tag line or lines shall be attached to loads where their use will not create a greater
hazard.
 All unused openings in floors, temporary or permanent, shall be securely planked over or
guarded.
 Erector shall have full set of tools in a secure condition on the elevated area in order to
insert bolts into the holes easily.
 Worker in high-elevated area shall not to walk on the member such as beams, girder,
without safety harness and hook it on the lifeline.
 After turning off electric power, welding machine, grinders and any other electric
machine, all electric cable shall be kept in one place, which is safe and secure.
 All the workers shall clean be working areas before they leave.
 All the working and lifting areas, access ways, dangerous openings must be adequately
and suitable lighted.
 Scaffold materials, tools and other objects, must not be thrown or tripped down from a
height. Potentially hazardous features should be identified that will conflict with health
and safety.
 If there is any overhead power lines near the proposed erection site, the electrical
specialist shall be consulted. Either the power lines should be made dead or suitable
precautions taken to prevent the danger.
 Certain activities or processes on or adjacent to the site may have the potential to
adversely affect the health and safety of workers on site. For example, noxious
gases, vapors or dusts may be given off from chimneys, stacks, tank vents and
ventilation ducts…etc. These may not cause a problem at ground level but may affect
steelwork erectors working at a height.
 A safety guard railing; Hard Barrier shall be installed approximately 42 inches high
around the periphery of all temporary-planked floors

10.3 Lifting and Rigging

 Column, girder, beam and bracing to be lifted by crane shall be tied with tag line or nylon
rope on both ends before lifting, shifting of location or for erection.
 Rigger designed as the banks man shall be authorized to instruct to the crane operator
before load and unload of structural members.

 Rigger shall use suitable shackles, Webbing sling, chain pulleys, wire sling with adequate
capacity.
 Where erectors have to work at height, provision must be made for safe means of access
to the connecting points and any other working places. Suitable platform such as man-
basket, Hydraulic ladder, and aluminum ladder shall be prepared for erection.
 Erector, bolting team shall wear safety harness, especially if they are working in high
elevated area; make sure that Full body safety harness is attached on main hoist or girder
which was erected already.
 Competent person must check the weight of the load and the load is fully distributed or
not.
 Crane outrigger must be fully extended & placed steel plates underneath of outriggers
for even surfaces to avoid toppling.
 While doing lifting operation if the load exceeds SWL crane indicates warning alarm
sound.
 Prior to start crane operation must be checked overhead lines.
 Competent Rigger must be used to operate lifting appliances.
 Competent signal man must be used to give signal to crane operator.
 Communication devices like walkie talkie provided to the crane operator and signal man.
 There is no interference in walkie talkie to avoid the miscommunication during the
operation.
 Appropriate precautions must be taken to ensure the stability of lifting appliances when
used on soft, uneven or sloping ground. These could include measures such as ground
leveling, use of mats, or hard standing.
 The load must not exceed the safe working load (SWL) of the lifting appliance Suitable
packing should be used to prevent the edges of the load from damaging the slings.
 Calculating the weights of components to assist in the estimation of safe crane capacities
and the location of cranes.
 Any load that approaches the safe working load of a crane should be carefully raised a
short distance and the operation stopped to check safety and stability before continuing.
 If more than one lifting appliance is used to raise or lower a load, a competent person must
supervise the operation which must be arranged so that no lifting appliance is loaded above
its safe working load or is made unstable
 If there is a tandem lifting there will be a two lifting crew teams must be appointed.
(Lifting crew members – Lifting Supervisor, Rigger Man and Signal Man)
 If it is a Tandem lifting two PTW must be applied for the two Cranes.
 Critical lifting plan is required for the tandem lifting.

11. GENERAL INFORMATION FOR INSTALLATION AND ERECTION SULPHAR STORAGE


SHELTER.

If no other requirements presented distribute packages in the same order and frequency as
the installation progress, this will keep down the space needed during the assembly. Out of
structural point of view the direction of installation is free of choice. Floor drawings with
maximum allowed surface loads must be provided by others. Check the floor drawings for
max surface loads above/close to trenches before situating the cranes. Use load spread
beams or heavy plates under the crane’s supports if required.

11.1 CHECKING THE ALIGNMENT OF THE FOUNDATION BOLTS

Columns are placed on in site installed foundation bolts installed by others, columns
bearing minor loads such as door frames shall be installed on chemical anchors type HILTI
or similar. At least a base line and a height reference should be provided by others.

Contractor making it possible to check the alignment of the in situ installed foundation bolts.

Grouting under base plates will be done by others (levelling grout 25 mm). This
operation should be done as soon as the structure is installed and preferably before the
outer fabric is installed.

11.2 LEVELING THE FOUNDATION BOLTS.

The foundation The foundation bolts must be leveled before starting to install columns. The
leveling can be done by placing steel spacers on the concrete until the correct level is
reached. The reference level is either presented as a value relative to a height fixation point
on the site or just simply as a value related to one of the foundations of the building. The
leveling can be done by means of an optical leveler or a Rotating Laser levelling device.
Other alternative to levelling is by placing an additional nut to every bolt head and adjusting
the nut until its top end represents the level of the lower surface of the footplate.

11.3. INSTALLATION OF COLUMNS

Check that the foundation bolt threads are free from damages, cleaned and greased.
Distribute the columns, bracings and purlins close to their respective fundament. Install the
bottom plate 925mmx925mm/1070mmx1070mm for the high column, and the hinge
connection plate at the top of the concrete wall.

Only the columns starting on ground level should be installed separately as single pieces,
the column starting on top of the concrete wall should be part of the pre-assembled frames
lifted as 3-arches in a package.

Start the installation with columns situated as components in a wind bracing field. Attach the
lifting ropes to the lifting eyes and install the columns to the foundation bolts. Before
disconnecting the lifting ropes, install the upper nuts to the anchor bolts and connect
stabilization wires to the first column until the second column is installed and connected to
the first one with purlins or/and wind bracing. Use a personnel access lift while disconnecting
the lifting eyes from the top of the column.
The first high column should be temporary supported with wires or similar in
“weak” direction until it is joined to the second and third column either through purlins or wind
bracing.
Approved survey reports should be maintained for each and every erected column as per
QCP requirements.

11.4. ROOF ELEMENTS INSTALLATION

Depending on the available space the roof sections can be pre-assembled units (2-3-5
arches) forming one lifting pack. Several lifting packs can be pre-assembled on ground and
lifted in a continuous sequence, making crane usage more effective. The principle for this
procedure is described in detail in document.

11.5. PRE-ASSEMBLY ON GROUND


The pre-assembly starts from the ridge elements. The first ridge element must be supported
from falling until the second element on either side is installed and they are firmly connected
with purlins.

The ridge elements should be connected to each other in this order:


1. Ridge beam
2. Buckling arms and purlins (upper and lower girder)
3. Cross arms (braces)

The connections between the ridge elements should be tightened as soon as the 3…5 set of
ridge elements are connected to each other. Make sure that the distance (c/c) between the
arches stays correct during the tightening stage. Finally, when the section is otherwise
ready, install the wind bracings, one end can bolt to the roof truss and the other end secured
with a rope until the section is lifted to its final destination. Possible part of the wind bracing
that falls between lifting sections (3 arches) will have to be installed once the frame is up by
using a small crane or telescopic handler.

Tightening order:
1. Buckling arms (lower)
2. Buckling arms including ridge beam (upper) and purlins
3. Cross arms (braces)
4. Wind bracing
The rest of the roof elements can be assembled using principally the same
methods as described above.
NB! Bolts are tightened to torque and to be inspected by suitable means (digital torque
wrenches/ torque wrenches). Final user to make regular inspections to bolts, inspection
timing and methods outlined in the maintenance manual for the buildings.

NB! After lifting specific number of arches, part of the cross arms installed needs to be
removed and used for preassembly of the following sections of the building.

Supporting frames for the ventilators are also advisable to be installed to as many sections
that possible on the ground.

11.6. LIFTING PREPARATIONS

Before starting to lift the roof sections the following preparations/checks shall be made:
- Weather forecast for the next 4 hours. (Wind speed < 15M/H).
- Study the lifting arrangement drawings
- Toolbox talk with the crew about the procedure and safety issues.
- Cranes refueled and checked
- Lifting slings etc. lifting aids available and in good condition.
- Connect a 20...30 m rope to the corner of the section –this will make it possible to
prevent the section from spinning around while lifting.

11.7. LIFTING AND INSTALLATION


The lifting sequence is a two-crane operation where two halves of the roof structure are
simultaneously lifted and jointed in the ridge center.

In the initial stage of the lifting the arch will take its shape and the lower truss connections
must be bolted one after one until they are all fixed. After this the final lifting can proceed.

Lift the sections above the columns and slide in the upper truss console to the
corresponding console on the main columns, use crowbars to make the final adjustments
and put bolts in the connection as soon as the holes are corresponding. Connect the purlins
to the previously lifted sections and the wind bracings to their connection points at the
columns. Do the final screw tightening when all elements in the roof section are placed on
their consoles.

11.8. CABLE STRUCTURES INSTALLATION

The gable structures can be installed when the rest of the structural steel is installed. Install
gable columns and their support purlins. Gable columns are divided in to two/three pieces
on order to fit in containers, commonly the column parts are joined together on ground
before lifting.

11.9. PENETRATION SUPPORT FRAMES INSTALLATION

The penetration support frames for cable rack and zipped doors should be installed after that
the main frame is installed but before the fabric installation. Also rest of the support frames
for the ventilators are advisable to be installed before the fabric.

The Installation of the membrane can start when the structural frame is installed. The
“sausage” should be rolled out (not opened) alongside the building. Make sure that the liner
is situated the right way, colored tags on the ends of the package should correspond with
the text markings on the installation drawing. Situate one crane alongside the building and
the other crane close to gable. Attach slings to the ends of the “sausage” and lift it
simultaneously until the lowest part is clearly above the roof, slightly swing the booms in to
the roof, until the “sausage” is about half way up the ridge. Lower it down to the roof and tie
it down before disconnecting the cranes.

The roof and wall membrane are to be divided in to 6 parts (between 50-60 m length) on the
Petcock building and 4 parts on the Sulphur building.

If the weather is suitable (wind speed not above 5 m/s ) and the forecast for the next 8
hours doesn’t indicate any changes the sausage can be opened and the installation work
performed.

11.11. ROOF POCKET TUBE INSTALLATION

The installation of the roof pocket tubes can be started after the liner is secured and
tensioned. Put the 120x60 tubes in their pockets and tension the pocket. The direction
of tension is towards the wall. The tension work must be repeated 2-3 times during the
installation phase while the membrane settles itself on the frame. The pocket tube
tensioning rod nuts are locked.

12. RISK ASSESSMENT AND MITIGATION

The appropriate Subcontractor HEMP (Hazard and Effect Management Process) procedures
shall explain the hazards and their effects control and recovery measures as per the approved
HSE management plan. JSA for the Erection works, attached as part of this method statement.
