    1. The Schools Division Office of Tarlac Province announces the list of qualified SEAMEO
    INNOTECH division scholars for Guro21 Course 1, LEADeXCELS and TEACHeXCELS.
    Classes will start on April 18, 2022, and end in July 2022.

    2. Attached is the Memorandum of Agreement (Scholarship Contract) of the scholars to


    be accomplished and submitted on this link

    SCHOLARSHIP CONTRACT

    I, ____________________________________ (NAME), Filipino, of legal age and with residence


    at _____________________________ (HOME ADDRESS), _________________ (POSITION) of
    ____________________________(SCHOOL) for being a scholar of the Schools Division Office
    of Tarlac Province with Southeast Asian Ministers of Education Organization Innovation
    and Technology (SEAMEO INNOTECH) as the Learning Service Provider, do hereby agree
    to adhere to the following terms and conditions:

    1. shall finish the course and comply with all the requirements prescribed by the
    SEAMEO INNOTECH, Quezon City, including the Revalida (validation) of the course
    outputs;

    2. That after having successfully completed the course, shall submit to the DepEd
    Schools Division Office of Tarlac Province a certified/validated photocopy of the
    Certification of Completion/Competence with a rating of “A” or “B;

    3. That in the event I fall short of the course requirements or fail to finish the course
    through my fault or willful neglect, resignation from the service, transfer to other
    agencies, voluntary retirement or other reasons/causes within my control, I shall
    be obliged to “refund” in full the registration/tuition fees and other expenses
    incurred by the Schools Division Office of Tarlac Province as the sponsor agency;
    and

    4. That after completion of the ____________________________________ (NAME OF


    PROGRAM/COURSE TITLE), shall render the compulsory service obligation as
    instructional supervisors in accordance with the Civil Service Commission
    Memorandum Circular (CSCMC) No. 44, s. 1992, equivalent to the period of
    scholarship indicated as follows:

    4.1 Duration: a fraction of a year not one (1) year exceeding six (6) months,
    but not less than two (2) months; and

    4.2 Service Obligation: one (1) year
