    The terms and conditions of our MCU services are as the following details:

    1. Price available for In-house clinic MCU.


    2. MCU In-house clinic available at MP BOP Clinic (BELTWAY OFFICE PARK) or WTC2 Clinic
    BELTWAY CLINIC: BELTWAY OFFICE PARK, ANNEX BUILDING, Ground Floor. Jl. TB. Simatupang Kav 41.
    Jakarta 12550
    WTC 2 CLINIC: WORLD TRADE CENTER 2, Lower Ground Floor. Jl. Jend. Sudirman, Kav. 29 Jakarta 12920
    3. Appointment shall be notified by Guarantee Letter at least 3 (three) working days before the MCU
    schedule and send by email before 15:00 WIB (email to: beltway.clinic@medikaplaza.com or
    wtc2.clinic@medikaplaza.com ; cc: mila.alfina@medikaplaza.com .
    4. For Appointment cancelation or reschedule please be informed at least 2 (three) working days before
    MCU and send by email before 14:00 WIB (email to: beltway.clinic@medikaplaza.com or
    wtc2.clinic@medikaplaza.com ; cc: mila.alfina@medikaplaza.com .
    5. List of MCU participants shall be received by Medika Plaza at least 1 week before the MCU schedule (if
    MCU on group basis)
    6. MCU participant shall have at least 10 hours fasting before the MCU schedule
    7. Thecresult of MCU will be done within 2 days and should be delivered at given address attached
    onclient form (for onsite MCU maximum 14 days after MCU)
    8. Term of Payment is 14 days after invoice received
    9. Quotation valid until 02nd November 2023
