    Good day to you and your good office.

    In the interest of the service, I have the honor to request your good office for transfer of station
    from Pangalangan National High School, as Teacher II to Tarece Integrated School for the following
    reasons to wit:

    1. That I am a bona fide resident of Brgy. Tarece, San Carlos City, Pangasinan.
    2. My mother underwent a surgery recently and needs my time and attention from time to
    time. Transferring to a school nearer our house will help me focus on my work and be there
    for my mother as and when needed. 
    3. That I will be spending little time, money, and effort in going to from my place of work if
    transferred in Tarece Integrated School.
