Purpose

This LP helps you learn the elements of a complete health history, interview a patient
to gather the data for a complete health history, analyze the patient data, and record
the history accurately.

Study Guide

After completing the reading assignment, you should be able to answer the
following questions. Answers should be written in a regular size notebook with your
name, and section. Please write legibly.

1. State the purpose of the complete health history.

2. List and define the critical characteristics used to explore each symptom the
patient identifies. (Patient should be a family member)

3. Define the elements of the health history: reason for seeking care; present health
state or present illness; past history, family history; review of systems; functional
patterns of living.

4. Discuss the rationale for obtaining a family history.

5. Define a pedigree or genogram.

6. Discuss the rationale for obtaining a systems review.

7. Describe the items included in a functional assessment.

Skills

The purpose of this component is to practice conducting a complete health history


on a family member.

1. Demonstrate knowledge of interviewing skills by: arranging a private, quiet,


comfortable setting; introducing yourself and stating your goals for the
interview; posing open-ended and direct questions appropriately; listening to the
patient in an attentive, nonjudgmental manner; and choosing appropriate
vocabulary that the patient understands.

2. Demonstrate knowledge of the components of a health history by: recording the


reason for seeking care in the person’s own words; eliciting all the critical
characteristics to describe the patient’s symptoms; gathering pertinent data for
the past history, family history, and systems review; identifying self-care
behaviors and risk factors from the functional assessment.

3. Record the history data accurately and as a reflection of what the patient believes
the true health state to be.
