    A logistic operator suffered a leg fracture when the stacker he was operating in reverse mode hit a trolley located behind him. This was caused by using a standing mode stacker in a limited space area without proper zoning for empty trolleys. To address this, the company updated its logistic golden rules to forbid using standing mode stackers in certain areas, defined zoning for empty trolleys, and implemented risk assessments for temporary layout changes.
