To the participants,
We, the researchers, invite you to participate in our survey titled “Factors Affecting the Choice
of Strand or Track of Grade 11 STEM Students”. This research is being conducted by selected
students of STEM. The purpose of this survey is to gather information and insights on the
various factors that influence students' decisions on the strand or track they choose in senior
high school. This information will be useful in understanding the students' perspective, and it
will also help educators and school administrators to provide better counseling, guidance and
support to students in making informed decisions about their education and career path.
Your participation is voluntary, and you have the right to not participate. If you choose to
participate in this survey, you may opt out at any time. There are no risks associated with
participating in this study. The survey collects no identifying information of any respondent. All
of the response in this survey will be recorded anonymously.
The survey will be conducted among the senior high school students in Oton National High
School. The survey will be anonymous and voluntary, and will take about 5-15 minutes to
complete. The survey will include questions about the factors that influenced the student's
decision to choose their current strand or track, their level of satisfaction with their choice,
and their perceptions of the support and resources provided by the school and community.
The survey will also provide students with an opportunity to share their own experiences and
insights.
By completing and submitting this survey, you are indicating your consent to participate in the
study. Your participation is highly appreciated.
