You are invited to participate in this survey on perspectives and suggestions of grade 10
students for improvements of academics in Sandiat National High School. This is a survey report
being conducted by grade 11-Emerald at Sandiat National High School.
This survey should take fifteen to twenty (15-20) minutes of your time approximately to
complete.
About the Study
This a survey report that shall serve as partial fulfillment of performance task in our class. This
survey aims to gather the perspectives and suggestions of grade 10 students to identify areas
for improvement in academics at Sandiat National High School. By understanding the
viewpoints of the students, the school administration and teachers can take their opinions into
account and implement measures to enhance the overall academic experience. The survey
intends to provide valuable insights that can contribute to the school’s continuous
improvement and the students’ educational development.
Participation
Please understand that your participation in this survey is voluntarily and you may, anytime,
refuse to take part in this survey report. In case you do wish to withdraw, please let anyone
from our group to know your wish to withdraw. You may freely decline in answering questions
that may be uncomfortable for you.
Benefits
You will not gain any material benefit in your participation in this survey report. However, you
will feel good helping us to reach our intention in completing this performance task and may
lead to further progress in how the school may improve by the perspectives and suggestions of
grade 10 students for improvements of academics in this report.

The answers to the survey will be protected and secured. We will not share your name, or any
information shall personally identify you. We assure you that any information that we receive
will be held in trust and confidence.
