Health-Perception/Health Management
PURPOSE: It focuses on ways to preserve health as well as the individual's
perceived state of health and wellbeing. 1.1Client Description of her health: The patient is 27 years old diagnosed with molar pregnancy, 3 pregnancies 2 live birth; G3P2, When the patient took a pregnancy test, the results were positive, and she reported having symptoms of nausea, vomiting, and Pica. 1.2Health Management: The woman admitted that she did not have a pregnancy checkup within three months and experienced painless vaginal bleeding. by convincing her to go to the neighboring clinic for a checkup, which led to her being admitted to the hospital. Nutrition and Metabolic Pattern PURPOSE: Describes pattern of food and fluid consumption relative to metabolic need and pattern indicators of local nutrient supply. 2.1 Usual Food Intake: This are the foods and beverages the patient usually intake. Breakfast – 1 cup of coffee and bread Lunch – 2 cups of rice and vegetable dish with (Gata) Snack – Biscuits and 1 cup of coffee Dinner – 1 cup of rice and fish 2.2 Usual Fluid intake (Type, Amounts) o Water – 5-8 glasses per day o Coffee – 2 cups per day 2.3 Any Food restrictions: According to the patient, she doesn't have any food allergies or dietary restrictions. 2.4 Any problem with ability to eat: Patient is able to eat normally and has no issues with eating. 2.5 Any supplements: Patient is able to eat normally and has no issues with eating. ELIMINATION PATTERN PURPOSE: It's focused on excretory patterns (bowel, bladder, skin). 3.1 Bladder Prior to admission, the patient has normal urine output, but once admitted, depending on how much fluid and water she consumes daily, she may not always be able to urine normally. Color: Light yellow Complain the usual pattern of urination: While urinating, neither a burning nor a painful tilt are present. 3.2 Bowel Usual pattern of Urination: The patient claims that she usually bowels in the morning with yellow, well- formed stools and also in the evening. 3.3 Skin (Condition) The patient's skin is in good condition and is tanned. 4.1 Usual daily/ Weekly activities: Leisure- (Paglalaba, paglilinis ng bahay, and pag babantay ng mga anak) Exercise - Patient doesn't exercise at all. 4.2 Any limitation of physical activities: Prior to the examination, she claimed that there were no restrictions on physical activity, but once a molar pregnancy was discovered, she was advised to stay in the hospital and rest. 4.3 History of Dyspnea or Fatigue Just before to becoming pregnant, the patient claimed she didn't easily get tired, but after being diagnosed with molar pregnancy, she began to feel fatigued and had dyspepsia. 5. SLEEP- REST PATTERN PURPOSE: Describe pattern of sleep, rest, and relaxation. 5.1 Usual Sleep Pattern: The patient claimed that while sleeping at home, she went to bed at precisely 8:00 pm and woke up at 7:01 am, but in the hospital, things are different because of the noise disturbance, making it difficult for her to get enough rest. She usually went to bed at the hospital around 11:00 pm and woke up at 5:00 am. 5.2 Any problem regarding sleep: As stated in sleep pattern, because of noise disturbance in the hospital, making it difficult to her to fell as sleep. 6. COGNITIVE-PERCEPTION PATTERN PURPOSE: Describe sensory-perceptual and cognitive pattern 6.1 Any deficits in sensory perception (hearing, sight, touch): The patient claimed that she has good sensory perception in all areas. 6.2 Ability to read and write. Any difficult in learning? The patient is capable of reading and writing, and she also has good comprehension skills. 7. SELF-PERCEPRION-SELF CONCEPT PATTERN PURPOSE: focused on the person's attitudes toward self, including identity, body image, and sense of self-worth. 7.1 What the client most concern about: money is one of their family's problems, the client is most worried about that. Also, she missed it so much that she is worried about her children at home. 7.2 Present Health Goal: The patient asked about her current health objectives, and the patient responded that she wanted to get better and be able to be discharged so that she could go home and take care of her children. 7.3 Effects of present illness to self: The patient's prolonged hospital stay gave the impression that he was bored, worn out, and restless. 8. ROLE RELATIONSHIP PATTERN: PURPOSE: It's focused on the person's roles in the world and relationships with others. 8.1 Language spoken and manner of speaking: The patient is responsive with a low soft-pitched voice tone and she use bicol language. 8.2 Significant person to client: Her children and family are what give the patient her strength to overcome her challenges in life. 8.3 Complain Regarding Family: As of the time of my interview, she has no complaints about her family, but her family's financial situation is the biggest issue. 9. SEXUALITY AND SEXUAL FUNCTION PURPOSE: Assessment is focused on the person's satisfaction or dissatisfaction with sexuality patterns and reproductive functions. 9.1 Knowledge of Sexual function The patient lacks understanding of how relationships work and family planning. 10. COPING STRESS TOLERANCE PATTERN PURPOSE: its focused on the person's perception of stress and coping strategies Support systems, evaluated symptoms of stress, effectiveness of a person's coping strategies. 10.1 Decision making ability Since the patient is an adult, she is capable of making decisions on her own, but she requires guidance in order to make wise and appropriate choices. 10.2 Management of Stress When a patient is under stress, she asks her husband for assistance, and having her kids around helps her to manage stress. 10.3 Expectation from nurse to provide comfort and security during hospitalization: The patient anticipates receiving the right medical attention and reassurance that she will recover and be released. 11. VALUE BELIEF PATTERN PURPOSE: It emphasizes the individual's values and beliefs. 11. 1 Source of Strength The patient claimed that her family, particularly her children, provide her with the strength to battle for her health. 11.2 Importance of God to client: The patient affirmed that God would assist her in overcoming her obstacles in life and in making a full recovery. 11.3 Religious practices (type and frequency) The patient occasionally goes to mass on Sundays despite her inconsistent attendance at it.
