INTRODUCTION

A routine Hospital Medical Record Meeting held on 15 TH October 2019 highlighted the importance of
standardizing the abbreviation used in patient’s clinical documentation in daily practice. A concensus was made
to produce manual of abbreviation and symbols for own perusal. With the cooperation and support from
Casemix Liaison Officer of various clinical departments, initial compilation was made followed with several
meetings to finalize and endorsed the abbreviation and symbols agreed upon for publishing and dissemination.
