That I, A, Filipino Citizen, of legal age, and with residence and postal address at X do
hereby name, constitute and appoint his wife, B, of legal age and with postal address at X, to
be my true and lawful attorney-in-fact for and its name, place and stead, to do and perform
the following acts and deed, to wit:
HEREBY GIVING AND GRANTING unto said attorney-in-fact full power and authority
to do and perform any and every act and things whatsoever requisite, necessary or proper to
be done in and about the premises as fully to all intents and purposes, as we might or could
do if personally present and acting in person, with full power of substitution and revocation;
and
HEREBY RATIFYING AND CONFIRMING all that said attorney-in-fact shall lawfully
do or cause to be done under and by virtue of these presents.
