Portfolio Format For Sy 2022 2023 2nd Sem.
The portfolio allows the student-intern to reflect on the most significant events
and experiences of their internship or work experience training. Student-intern
integrates these reflections into a comprehensive internship portfolio which
both showcases their specific achievements in the workplace and analyzes
the quality of their learning throughout the internship period. Contents are
normally compiled as a soft-bound booklet. All submissions must be well-
organized (i.e. with cover page, table of contents, tabbed, etc.), typed and
otherwise professionally presented. The front cover and title page must
include: complete name, course, year and section of
the student-intern, a title – usually the SIP Center/Cooperating School Name,
and the semester/year taken. The following suggestions are not intended to
be all inclusive but are only guidelines for the final Internship Portfolio. The
portfolio MAY include but is not necessarily limited to the following content:
o Introductory Statement
Give the name of the SIP Center/Cooperating School and
briefly describe the type of company/school that it represents
Provide a map showing the location of the SIP Center/
Cooperating School (use QF-SIP-03 form)
Description and understanding of the SIP Center/
Cooperating School’s governing authority and managing
authority (i.e. legal basis for the establishment, such as laws
of authorization, enabling laws, ordinances, charter, etc.)
o History/Development
From a historical perspective, how has the SIP Center/
Cooperating School evolved into its present form?
What were the stages of development and growth?
When and why it was originally founded (including the subunit within
which the intern is assigned) in its present location?
o Purpose/Philosophy
What is the mission and/or vision statement of the SIP
Center/Cooperating School?
What is the SIP Center/Cooperating School’s philosophy or overall
goals as they relate to providing products or services to its clientele?
o Personnel
Provide an organizational chart and label the names of the
key executive officers, managers, etc. Also show your place
in the organization
Give a brief description of for the individuals that an intern might
interact with on a regular basis, and include whether they are full-time,
part-time, seasonal and/or volunteers.
