    STUDENT INTERNSHIP PORTFOLIO

    The Student Internship Portfolio encapsulates the cumulative experience of


    the Student Internship Program. It is a collection of materials developed and
    assembled by the student-intern as evidence of his/her performance and
    acquired skills and knowledge over the course of the internship program. This
    might include samples of the intern’s work found in letters, memos,
    photographs, videos, reports or brochures, as well as evaluations written by
    their clients, colleagues or immediate supervisors.

    The portfolio allows the student-intern to reflect on the most significant events
    and experiences of their internship or work experience training. Student-intern
    integrates these reflections into a comprehensive internship portfolio which
    both showcases their specific achievements in the workplace and analyzes
    the quality of their learning throughout the internship period. Contents are
    normally compiled as a soft-bound booklet. All submissions must be well-
    organized (i.e. with cover page, table of contents, tabbed, etc.), typed and
    otherwise professionally presented. The front cover and title page must
    include: complete name, course, year and section of
    the student-intern, a title – usually the SIP Center/Cooperating School Name,
    and the semester/year taken. The following suggestions are not intended to
    be all inclusive but are only guidelines for the final Internship Portfolio. The
    portfolio MAY include but is not necessarily limited to the following content:

    SECTION A: Pre-Internship Documents. This portion of the portfolio


    includes student-intern’s requirements that must be submitted prior to the
    SIP Center/Cooperating School, a copy of which should also be provided
    to the SIP Supervisor/Teaching Internship Supervisor. These requirements
    are as follows:
    o Copy of the Student Internship Program Orientation Certificate of
    Attendance
    o Copy of Official Registration Form marked as Officially Enrolled
    o Application Letter addressed to the Department Head requesting to
    be included in the Student Internship Program
    o Application Letter addressed to the appropriate authority of the SIP
    Center/Cooperating Teacher
    o Resume with Passport size picture (student-intern must be in business
    attire
    in the photo)
    o NBI Clearance
    o SSS E-1 form
    o Medical Certificate
    o Copy of the Endorsement Letter duly noted and received by the SIP
    Center Supervisor/Cooperating Teacher
    o Duly accomplished and signed SIP Forms
    o (whenever needed) A copy of the executed MOA/MOU between the
    University and SIP Center/Cooperating School.
    o Other requirements needed by the SIP Center/Cooperating School
    SECTION B: SIP Center/Cooperating School Profile. This is also known as
    Company Profile. This portion should provide an in-depth overview of the SIP
    Center/Cooperating School (or department where student-intern is
    assigned if the SIP Center/Cooperating School is exceptionally large). This
    section should include:

    o Introductory Statement
    Give the name of the SIP Center/Cooperating School and
    briefly describe the type of company/school that it represents
    Provide a map showing the location of the SIP Center/
    Cooperating School (use QF-SIP-03 form)
    Description and understanding of the SIP Center/
    Cooperating School’s governing authority and managing
    authority (i.e. legal basis for the establishment, such as laws
    of authorization, enabling laws, ordinances, charter, etc.)
    o History/Development
    From a historical perspective, how has the SIP Center/
    Cooperating School evolved into its present form?
    What were the stages of development and growth?
    When and why it was originally founded (including the subunit within
    which the intern is assigned) in its present location?
    o Purpose/Philosophy
    What is the mission and/or vision statement of the SIP
    Center/Cooperating School?
    What is the SIP Center/Cooperating School’s philosophy or overall
    goals as they relate to providing products or services to its clientele?
    o Personnel
    Provide an organizational chart and label the names of the
    key executive officers, managers, etc. Also show your place
    in the organization
    Give a brief description of for the individuals that an intern might
    interact with on a regular basis, and include whether they are full-time,
    part-time, seasonal and/or volunteers.

    SECTION C: Overview of the Student Internship Program


    o Internship goals and objectives (use QF-SIP-09 SIP Training Plan)
    o Summary of Work Responsibilities (job description, assignments,
    projects)
    o Professional Development
    Orientation Process and Sessions (in the SIP Center/ Cooperating
    School)
    Workshops, Conferences, Seminars, Meetings, etc.
    In-service Training
    o Other examples of Work Completed
    Administrative work performed
    Correspondence prepared
    Photographs of events, etc.
    SECTION D: Internship Reports Section
    o Daily Activity Log and Journal (use QF-SIP-04)

    o Performance Evaluation Forms (use QF-SIP-05/QF-COE-05)


    o Student Evaluation of SIP Center/Cooperating School (use QF-SIP-08)
    o Other SIP forms duly accomplished and signed

    SECTION E: Documents from the SIP Center/Cooperating School


    o Student-intern should provide one original and two (2) photocopies
    of the certificate of completion which should be printed in the SIP
    Center/Cooperating School’s letter head/logo paper.
    o Student-intern must also submit his/her Daily Time Record (DTR)
    duly signed by the SIP Center Supervisor/Cooperating Teacher

    SECTION F: Appendices. These are items, photos, and other relevant


    documents pertaining to the internship or work experience and include
    outputs that are considered as alternative home-based activity as
    assigned to the student intern.
