    AFFIDAVIT OF DISCREPANCY

    I, JOHN EARL G. QUION, of legal age, Filipino, single, and residing at 2637 Lico Street
    Tondo Manila, after having been duly sworn to in accordance with law, hereby depose and
    state:

    1. That I was born on March 6, 1994 in Manila and my parents are MERCY G. QUION and
    DANTE A. QUION ;

    2. That in my Certificate of Live Birth issued by the Philippine Statistics Authority under
    Local Civil Registrar No. 94-15376, her name was recorded as JOHN EARL G. QUION;

    3. That JOHN EARL G. QUION is the name known by my relatives and friends and this
    is the name appearing in my school records and other pertinent legal documents bearing
    my signature;
    4. That in my E1 Form issued by Social Security System (SSS) my name was recorded as
    JOHN EARL G. QUION but in their Database my name was recorded as JOHN CARL
    G. QUION :

    5. That the name JOHN CARL G. QUION and the name JOHN EARL G. QUION refers
    to one and the same person;

    6. That I am executing this affidavit to attest to the truth of the foregoing facts and
    circumstances and for whatever all legal intents and purposes this instrument may
    serve.

    IN WITNESS WHEREOF, I have hereunto set my hand and affixed my signature on this 2ND
    day of September, 2019 in the City of Manila, Philippines.
