Lec 1

Definitions:

1. Crown and Bridge: It's a branch of dental science that deals with restoration of
damaged teeth with artificial crown and replacing the missing natural teeth by
fixed prosthesis [Fixed partial denture].

2. Crown: It's a fixed artificial restoration of


the coronal portion of a natural tooth .It must
restore the morphology, contour and the
function of the tooth and should protect the
remaining tooth structures from further
damage.

3. Bridge: It's a fixed prosthesis constructed to


replace the missing [one or more] teeth .The
tooth that gives support to the bridge is called
an abutment tooth.

Components of the bridge:

l. Retainer: It's the part that covers the abutment tooth, which could be major or
minor.

2. Pontic: It's the part of the bridge that replaces the missing tooth, which is the
suspended part of the bridge. It occupies the position of the missing natural tooth.

3. Connector: It is the part that connects the retainer to the pontic, which could be
fixed or movable connector.

When the retainer is attached to a fixed connector it's a major retainer, but when it
is attached to a flexible [removable] connector it is called minor retainer.
Bridge classifications:

1. Fixed-fixed bridge: is the most commonly


used design of bridge both anteriorly and
posteriorly. In this type the pontics are
connected rigidly to the retainers at both
ends of the bridge by solder joint
connectors
2. Cantilever bridge:
a. Simple cantilever bridge: which consist
of one or two retainers join with a pontic
that replaces a missing tooth. It is
specially used for aesthetic purpose
(missing lateral incisor or canine).
Usually in this type of FPD there is no
other retainer at the other side of FPD.
b. Spring cantilever bridge: the pontic is
connected to a gold bar that is also
connected to a retainer which is away
from the pontic.
It is used when we have a missing maxillary central or lateral incisor on
one side with the present of diastema and space of missing tooth is very
difficult to be replaced by fixed-fixed bridge.

3. Fixed removable bridge: the


pontic is attached to fixed retainer
on one side while the other side is
a movable joint that connected
with the other retainer.
It is used in cases of tilted
abutment specially in posterior region.
Clinical consideration for bridge construction
1. Oral hygiene of the patient: high caries index and bad oral hygiene are
contraindicated for making a bridge.
2. Age of the patient: crown and bridge cannot be made under age of 18
years due to large pulp and continuous eruption of the teeth.
3. Number of missing teeth: fixed bridges are useful to replace single tooth
or 2-3 rather than large no. of missing teeth because the more missing
teeth the more abutments are joined to be included with the bridge.
4. Condition of the abutments: periodontal condition of the abutment is very
important. For example lateral incisor alone can not be used to replace a
missing canine; other teeth should be included (premolar).
Also the tooth with large clinical crown and sound root is considered as
good abutment.
