GENERAL INSTRUCTION:
The purpose of this form is to determine objectively how well an employee is performing
in his/her current position. The form is based upon a consideration of all the major
functions a staff member performs.
INSTRUCTION:
Use the following rating scale in making your judgement. Check the box that correspond
to your answer.

5 Excellent (consistent exceeds standards)
4 Outstanding (frequently exceeds standards)
3 Satisfactory (generally meets standards)
2 Needs improvement (frequently fails to meet standards)
