    AFFIDAVIT OF LOSS
    I, ANLENE S. OLAGAN, of legal age, single, Filipino Citizen, with
    residence and postal address at Lubas, La Trinidad, Benguet, Philippines, after
    having been duly sworn to in accordance with law do hereby deposes and
    states, that:

    1. I am an applicant for Philippine Passport with the Department of Foreign


    Affairs (DFA)-SM City Baguio, Luneta Hill, Baguio City;

    2. When I applied for the said Passport, they issued me Official Receipt (OR)
    or Electronic Receipt;

    3. Aside from the said receipt under my name, I had also the receipt of Mr.
    Jether Jeph Deban Punasen and I kept both receipts in my bag;

    4. On December 22, 2023, I went to Burnham to have some coffee break. I


    left my bag for a while in one of the seats, however, when I returned
    back, I was shocked to see my bag was no longer in the seat where I left
    it, it was taken by unidentified person;

    5. I exerted diligent effort to find my said bag containing the said Electronic
    Receipts and other importants belongings, however, I failed to find it,
    hence, considered forever lost;

    6. The said receipts were not given nor pledged to anyone for any purpose
    whatsoever, nor were taken or confiscated by the authorities for violation
    of any law;

    7. I am executing this affidavit to attest to the truth of my foregoing


    statements and for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 12th day of


    January, 2024 at Baguio City, Philippines.
