1. Hi, I’m one of the pharmacists on duty, how can I help?
2. Is the request for yourself? 3. Could you describe the symptoms? 4. How long have these symptoms go on for? 5. How much have they been vomiting? 6. Are they voiding? 7. Are eyes sunken, lethargic, cold skin, skin turgor test? 8. Any other symptoms such as blood in stool, abdominal pain, fever, diarrhea etc? 9. Have you tried anything so far? 10. Any recent close contacts to anyone unwell? 11. On any meds/otc/herbals/supplements? 12. Allergies? 13. Med conditions? 14. Pregnant or breastfeeding?
