Smell scent with each nostril

separately & eyes closed.

ENSURE adequate lighting. Ask PT

to read from a reading material
OPTIC acuity held at a distance of 14 IN.

SHINE PENLIGHT TO ASSESS PERRLA


(PUPILS EQUAL ROUND REACTIVE TO
EYE LIGHT & ACCOMODATION)
IIi MOVEMENT:
Reaction: Shine penlight into each eye &
PUPIL SIZE & observe response of pupil. Shine light
OCULOMOTOR
REACTIVITY on pupil again & observe response of
other pupil.

accommodation: "Can you focus on my


penlight as I bring it to your nose?"

Hold a penlight 1 ft. in front of


pt's eyes. Ask pt to follow the
Iv Visual movements of your penlight
acuity with their eyes only. Move the
TROCHLEAR penlight upward, downward,
sideward & diagonally.

MOTOR: observe the strength and symmetry of


CHEWING movement when PT clenches teeth.
v & FACIAL
SEnSORY: instruct PT TO CLOSE EYES, USE cotton
Trigeminal SENSATION
swab TO lightly touch FACE, FOREHEAD & CHIN.
iNSTRUCT PT TO SAY, "NOW" EACH TIME THEY FEEL IT.

ASK PT to follow your


finger as you move it
vi LATERAL EYE
from midline toward
MOVEMENT the pt’s ear on one side
ABDUCENS
& then the other.

Ask PT to smile, show


teeth, close both eyes,
vii FACIAL puff cheeks, frown &
EXPRESSIONS raise eyebrows. OBSERVE
facial symmetry & strength of
facial muscles.

TEST EACH EAR INDIVDUALLY:


INSTRUCT PT TO OCCLUDE ONE EAR
VIII HEARING &
& WHISPER A PHRASE OR COMBO OF
NUMBERS/LETTERS (ex: j-k-8-9) IN
Vestibulo- BALANCE THE OTHER & instruct pt to
cochlear repeat phrase. test balance by
asking pt to stand w/ feet
together & eyes closed.

ask pt to open mouth & say, "ah."


Ix gagging & note symmetry of upper palate,
Glosso- ask pt to swallow, uvula &
swallowing
pharyngeal tongue should be midline.

Use tongue blade to touch


pharynx &
x larynx
pt's posterior pharynx;
observe for a gag reflex
vagus sensation followed by a swallow.

ask pt to shrug shoulders


shoulder
against resistance from your
xI shrugging & hands & turn head to side
accessory head rotation against resistance from your
hand. Repeat for other side.

ask pt to stick out their


XII tongue
tongue at midline & move it
side to side. Deviation to
hypoglossal movement one side or the inability to
stick out the tongue are
abnormal findings.
