    technicians for Roof installation activities at site :
    1) Welder – 04 Nos
    2) Gas cutter – 02 Nos

    3) Rigger – 02 Nos
    Time Sheet will be shared on a daily basis to Metco site
    team and the same will be certified by METs representative.
    Detection will be applicable from METs
    METCO agreed Mr. Samuel will be present at site from
    2 morning TBT to until the end of the day to manage site team METs
    properly
    Mr. Nayak (METCO), Welding Foreman need to do OQ
    3 medical and Submit Gate pass documents immediately to METs
    guarantee his presence at site for welding
    Mr. Atul (METCO), Needs to update his lifting supervisor
    certification, as two cranes will be used at the site and he will
    4 METs
    be responsible for supervising the lifting operations and
    granting permits for the second crane
    BG Ordered Required Welding Electrodes for this project
    5 and the same will be provided to METs. Detection will be BG / METs
    applicable from METs
    Phase 2 beams Fabrication will be since BG carried out as
    6 per Metco mail request the weight age will be deducted their BG / METs
    accounts
    Metco agreed the Roof Fabrication will be completed as per
    7 METs
    the schedule dated 5th of September
    Metco should ensure site PPE as well as frequent
    8 absenteeism at site Half day show off duty should be METs
    controlled
    Currently working in Site Metco deployed riggers card
    existing third party not approved by OQ since the incident is
    9 METs
    happened they have concern in competency , Hence re
    validation to be done OQ approved third party agency
