SUBJECT : Spot Report re: Shooting Incident

DATE : November 5, 2019

1. Reference: Blotter Entry No. 361, page 417, dated November 5, 2019
with same subject as above.

2. This pertains to the shooting incident that transpired at about 1: 00 PM


of November 5, 2019 at Rizal Street, Barangay Sta. Cruz, Magalang, Pampanga.
Reported by Maria Dates Domingo, 44 years old, married, housewife, Filipino,
female and residing in J and J Apartment, 50 Rizal Street, Barangay Sta.Cruz,
Magalang, Pampanga wherein the victim identified as Fernando Sagatan Domingo,
45 years old, married, driver, Filipino, male also residing at the aforementioned
address. Upon received of the report PPSg Danny Flores and Patrolman Carlo
Reyes.

3. Investigation conducted disclosed that while the victim was washing his
car he was shot three times on the head by the unknown suspect (at about 5’6” in
height, wearing black helmet with bonnet, white t-shirt and maong pants as stated
by the witness). After the crime suspect immediately fled riding his black motorcycle
without plate number towards unknown direction. The victim immediately brought at
St. Gabriel Hospital and Medical Center, Magalang by the responding police officers,
but declared dead on arrival (DOA) by Dr. Eric Mendoza the attending physician.

4. SOCO team from Angeles City Crime Laboratory Office requested and
conducted scientific investigation. Recovered from the crime scene were three empty
shell of suspected caliber .45. Further, this office requests to conduct Autopsy to the
cadaver and Ballistic examination on the recovered empty shells. Follow up
operation currently conducted by this office to identify and locate the suspect.
