Answer the following questions:

1. Using the acronym, “A.C.T.I.O.N,” identify and explain the sequence the tool recommends you
follow to address all medication problems? (6 marks)

2. For each situation below, answer the following questions:

Situations:

1. Your client has Parkinson’s disease. He tries to administer his own oral medication. His wife,
who usually helps him, is often not home for his afternoon dose because your visit is her
respite time.

A. In your own words, explain the main problem/challenge faced in this situation. (2 marks
for each situation)

➢ In this case, the client, diagnosed with Parkinson's disease, may encounter challenges
in locating or managing his medication independently. Given that his usual caregiver,
his wife, is unavailable to assist with his afternoon dose, there is a risk of the client
either missing the dose entirely or inadvertently taking the wrong dosage.

B. What section in the “Medication Problem A.C.T.I.O.N. Plan” resource would you turn to
when addressing the situation? (1 mark for each situation)

➢ This problem can be solved by going to section C (barriers and other issues) in the
action plan.
C. What action would you take to assist your client? Record your answer for part “c” as if you
are documenting in the client’s record. (3 marks for each situation)

➢ - 04/19/2024 @1500H: The PSW observed that the client requires assistance with his
oral medication due to his wife's frequent unavailability during his afternoon dose.
To reassure the client that assistance is forthcoming, the PSW will contact the
supervisor to report the medication challenges, emphasizing the client's inability to
manage his medication independently. Additionally, upon the wife's return home,
the PSW will discuss this issue with her to ensure a solution is promptly implemented
and the client's medication needs are adequately addressed. ------- W, T. PSW

2. Your client is 84 and has rheumatoid arthritis. Each week the RN distributes her medication for
each day in a docette. Today your client’s joints seem very inflamed and she asks you to open
her docette for her.

A. In your own words, explain the main problem/challenge faced in this situation. (2 marks for
each situation)

➢ In this case, the client, who is managing rheumatoid arthritis with medication, is
experiencing significant joint inflammation, signaling a shift in his health status that
warrants consultation with a physician for further assessment and guidance.

B. What section in the “Medication Problem A.C.T.I.O.N. Plan” resource would you turn to
when addressing the situation? (1 mark for each situation)

➢ This problem can be solved by going to Section D (change in client Condition).

C. What action would you take to assist your client? Record your answer for part “c” as if you
are documenting in the client’s record. (3 marks for each situation)

➢ 04/19/2024 @1500H: Assisted the client in opening the medication docette and
offered water to facilitate medication intake. Notified the supervisor of difficulties
encountered with opening the docette. Reported the client's condition of severely
inflamed joints to the supervisor. ------ R.P, PSW Student
3. According to the care plan for a new client, he is on an anti-inflammatory medication that has
been prescribed to be taken bid. When you try to remind him to take his medication, he
refuses, claiming he is fine.

A. In your own words, explain the main problem/challenge faced in this situation. (2 marks for
each situation)

➢ In this case, the client has been prescribed an anti-inflammatory medication to be


taken twice daily, yet he adamantly refuses, asserting that he feels well. However, it
remains uncertain whether his refusal stems from genuine well-being or serves as an
excuse to avoid medication. Failure to adhere to the prescribed treatment could
potentially exacerbate his condition, as untreated inflammation may lead to
increased swelling and pain.

B. What section in the “Medication Problem A.C.T.I.O.N. Plan” resource would you turn to
when addressing the situation? (1 mark for each situation)

➢ This issue can be addressed by going to section C (barriers and other problem).

C. What action would you take to assist your client? Record your answer for part “c” as if you
are documenting in the client’s record. (3 marks for each situation)

➢ 04/19/2024 @1500H: After assessing the situation, the PSW engaged in a discussion
with the client to understand the reasons behind his refusal to take medication,
ensuring the client's perspective was honored. Subsequently, the PSW informed
Nurse Emma, the supervisor, about the client's refusal to adhere to medication.
Efforts were made to emphasize to the client the importance of medication
adherence and explore alternative strategies. Follow-up will be essential to monitor
the client's well-being and address any additional concerns that may emerge. ------- T,

4. While reading the label on your client’s medication, you notice that the strength of the
medication is different from the care plan. Your client claims that his meds were changed over
the weekend after visiting the medical clinic.

A. In your own words, explain the main problem/challenge faced in this situation. (2 marks for
each situation)

➢ In this case, there is a discrepancy between the medication strength and the care
plan. The client asserts that the medication was altered during a recent clinic visit, of
which I was unaware. However, as the most recent care plan is unavailable, there
exists uncertainty regarding the accurate dosage to be administered.

B. What section in the “Medication Problem A.C.T.I.O.N. Plan” resource would you turn to
when addressing the situation? (1 mark for each situation)

➢ This issue can be addressed by going to section E (Change in client medication).

C. What action would you take to assist your client? Record your answer for part “c” as if you
are documenting in the client’s record. (3 marks for each situation)

➢ 04/19/2024 @1500H: The potency of the medication provided clinically differed from
what was outlined in the care plan. I notified the supervisor to communicate the
adjustments made to the client's medication strength. Additionally, I engaged with a
family member to verify their awareness of the change while cross-referencing the
label. It is imperative to conduct a follow-up during the next visit to confirm the
