INTERNSHIP AGREEMENT/CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Internship Agreement made and executed by JESABEL M. BARTOLOME, a Bachelor of


Science in Information Technology student of the Isabela State University - Jones with
principal office address at Barangay 1, Jones, Isabela, Filipino of legal age, hereafter referred
to as STUDENT INTERN.

- and -

COMELEC JONES, represented by JERICO JOHN R. CORTEZ with a principal office address at
Barangay 1, Jones, Isabela, herein referred to as HOST TRAINING ESTABLISHMENT (HTE).

This training period will begin on February 26 2024 and will end on May 24 2024.

- WITHNESSETH -

THAT, the Isabela State University – Jones Campus of which school the Student Intern is a
student, has requested the Host Training Establishment to allow Student Intern to undergo
internship for a minimum of Four Hundred Eighty – Six hours (486) as part of his/her
academic requirement.

THAT, the Host Training Establishment is willing to accommodate the request of the Isabela
State University – Jones Campus and to allow the Student Intern to undergo practical
training.

The following terms and conditions shall govern this agreement:

1. The HOST TRAINING ESTABLISHMENT shall provide the STUDENT TRAINEE


orientation and training on the areas pertinent to his / her line of specialization.

2. The training agreement shall not, in any way, constitute an employee-employer


relationship and the STUDENT TRAINEE shall leave the HOST TRAINING
ESTABLISHMENT free and harmless from any demand, claim or complaint,
whatsoever arising from this training agreement, except in cases of gross negligence,
malicious acts and criminal acts by the HOST TRAINING ESTABLISHMENT or any of its
officers and employees.

3. The HOST TRAINING ESTABLISHMENT may only require STUDENT TRAINEE to report
on Monday - Friday. The training hours must only be between 8:00 AM and 5:00 PM
for weekdays and 7:30-12:00 for weekends or as need arises. A STUDENT TRAINEE
should be given one hour of lunch break (12:00 PM – 1:00 pm) and at least 15-30
minutes break during morning and afternoon schedules. In case of HIGHER
EDUCATION INSTITUTION suspensions/holidays, the STUDENT TRAINEE may be
allowed to report to the HOST TRAINING ESTABLISHMENT after the approval of the
SIPP Coordinator.

4. The status of the STUDENT TRAINEE while on training shall be that of STUDENT
TRAINEE. As such, he/she shall not be entitled to any compensation and to any of the
benefits accorded to an employee.

5. The HOST TRAINING ESTABLISHMENT may grant the STUDENT TRAINEE a training
allowance based on HOST TRAINING ESTABLISHMENT policies.
6. The STUDENT TRAINEE shall conform to all the rules and regulations of the HOST
TRAINING ESTABLISHMENT while on training.

7. The employees of the HOST TRAINING ESTABLISHMENT should maintain a strict


professional relationship with the STUDENT TRAINEE. Any personal involvement with
the STUDENT TRAINEE is not allowed

8. The STUDENT TRAINEE shall not divulge any information that he/she may have access
to, and any such information will only be used for academic purposes.

9. Both the HOST TRAINING ESTABLISHMENT and the STUDENT TRAINEE have the right
to pre-terminate the on the-job training if:

a. the STUDENT TRAINEE does not show the required interest, maturity, or
discipline during the training period, or if there is serious misconduct

b. the HOST TRAINING ESTABLISHMENT does not provide the kind of responsible
training as agreed upon.

The terminating party will inform the Student Internship Program of the Philippines (SIPP)
Coordinator before any decision to terminate is made and finalized. The SIPP Coordinator
will intervene in order to rectify the situation in the interest of all parties concerned.
