FOR INCOMING GRADE 11 STUDENT APPLICANTS


QUALIFICATIONS:

1. Graduating Junior High School Students of the school year preceding the academic year
opened for admission;
2. Passer of the Philippine Educational Placement Test (PEPT), or Alternative Learning
System (ALS)
Dual citizen and non- Filipino Grade 11 applicants including:
International student who graduated in Philippine high schools.
Dual citizen who declares Filipino citizenship. Enrollment Procedure
Permanent Resident with Alien Certificate of Registration. and Requirements
REQUIREMENTS:
Basic Education
For Incoming Grade 11 Applicants:
Senior High School
1. Original copy of Grade 10 report card with the applicant’s eligibility for admission to
Freshmen
Grade 11 duly signed by the school principal, but if your previous school has not yet
released your final report card, a certified true copy of Grade 10 report card from 1st to Transferees
3rd quarter will be present and uploaded in the online application temporarily for
International Students
evaluation.
2. Original copy of the Certificate of Good Moral Character (dated not earlier of March) Cross-Enrollees
from Grade 10 (with school seal) Graduate School & Juris Doctor
3. a. Original PSA Birth Certificate
b. Two (2) photocopies of PSA Birth Certificate. Doctor of Pharmacy
4. a. ESC Voucher Certificate (for those graduates in private schools who are DepEd
Voucher Program recipients).
b. Two (2) photocopies of Grade 10 report card (for those graduates in public schools
and DepEd Voucher recipients).
5. 2pcs. 2x2 ID picture white background wearing corporate attire with name tag. School Fees

For INTERNATIONAL STUDENT Applicants:
Senior High School
1. Letter of Application to enroll addressed to the Senior High School Principal. Indicate
Nursery, Kinder, Elementary and
the strand that the applicant intends to enroll and name of school last attended.
Junior High School
2. 2pcs 2x2 ID picture white background wearing corporate attire with name tag
3. Original copy and Two (2) photocopies of the Report Card
4. O!cial Transcript of Records/ Scholastic Records duly authenticated by the
Philippine Embassy or Consulate in the students’ country of origin or legal residence.
5. a. Original Copy of Birth Certificate/Home Register Scholarships
b. Two (2) photocopies of Birth Certificate/Home Register
6. Original copy of Certificate of Good Moral Character
7. Photocopy of passport data page (page containing students’ full name, date and place Scholarships for Freshmen
of birth, passport number and photo). must be duly authenticated by the Philippine Students
Foreign Service Post.
Academic Scholarship Program
8. Original and 1 photocopy of Special Study Permit from the Bureau of Immigration
Revised Policy and Implementing
9. Original and photocopy of Evaluation from the Department of Education (DepEd)

For those students who graduated from abroad, all documents must be duly authenticated/
apostille stamped from the Philippines Embassy or Consulate of the country where the school
is located

APPLICATION PROCEDURES

STEP 1: ONLINE APPLICATION

1. Visit the website www.adamson.edu.ph, click the Senior High School Tab for the online
application of enrollment.
2. Once logged in, students will have to accomplish their student profiles and give their
consent to the data privacy notice of Adamson University.
3. Applicants for STEM Strand.
No grades below 80 for Science, Math, Computer, and English subjects for each
quarter
No failed grades in all subjects for each quarter
4. Applicants for ABM and HUMSS Strands
No failed grades in all subjects for each quarter
5. Upload the scanned back to back copy (*jpeg, *jpg, *png, or pdf format) of SF9 (F-138)
of your 1st to 3rd Grading grades for initial evaluation.

For ALS or PEPT passers upload scanned copy of Certificate of Rating and diploma.

6. Once approved, the student applicant shall be given his/her applicant number.

STEP 2: REGISTRATION

1. Submit personally the admission requirements in an original hardcopy and present the
screen shot of application number in the Adamson University Senior High School
Admission Computer Laboratory 16 near University Store.
2. The SHS Admissions O!ce accepts documents during o!ce hours from Monday to
Friday, 9:00AM To 4:00PM. The o!ce is close on weekends and during holidays, and on
days when there is a suspension of classes declared by the school or by the National or
City Government of Manila.
3. The SHS Admissions o!cer shall issue student number to the applicant.
4. Using your assigned student number, the nonrefundable down payment of Php5,000
can be paid at the Adamson University Cashier.
5. Return to the Computer Laboratory 16 and present the down payment receipt to the
Senior High School Admissions o!cer for encoding of section.
6. After Encoding, you will be provided with your Certificate of Enrollment (Registration
Form) with Assessment of Fees and
7. Proceed to the University Store for your school uniform concerns.
8. Purchasing of books shall be announced on or before the opening of the First Semester.
9. Congratulations! You are now o!cially an Adamsonian Student!

FOR GRADE 12 TRANSFEREE APPLICANTS


QUALIFICATIONS:

The applicant must have completed the Grade 11 courses from his/her previous school.

REQUIREMENTS:

For Grade 12 Transferee applicants:

1. Original copy of SF9 Grade 11 report card with the applicant’s eligibility for admission to
Grade 12 duly signed by the school principal.
2. Original copy of the Certificate of Good Moral Character (dated not earlier of March) from
Grade 11 (with school seal)
a. Original PSA Birth Certificate
b. Two (2) photocopies of PSA Birth Certificate.
3. Original Grade 11 SF10 (F137-A) Senior High School with attachment of certified true copy
of SF10 (F137-A) Junior High School (for students who are o!cially enrolled in Adamson
University).
4. 2pcs 2x2 ID picture white background wearing corporate attire with name tag

For INTERNATIONAL STUDENT Applicants:
