Learning Task 9: Assisting My Cooperating Teacher (s) in

Accomplishing School Forms


Performance Task 1:
Assisting the Cooperating Teacher in Accomplishing School Form 1
Class Register
This is the DepED template of SF 1 Class Register. Study how it is
accomplished by requesting your Cooperating Teacher to show you a sample.
Put at least five (5) entries into the template.
DATA ELEMENT DESCRIPTION
School ID A six (6) digit-number assigned to a school recognized in EBEIS
School Year The prescribed period of time when schools offer daily instruction.
It covers 10 months of regular schooling starting month of June
(Beginning of school year) up to the month of March (End of School
Year) of the following year.
School Name Official name of the school as registered in DepED and EBEIS.
Grade Level A degree/stage of a learner classified according to age and
progress.
Section A group of pupils/students convened together to receive instruction
in a given course of subject.
Learner Reference Twelve (12) digit number which the pupil, student or learner shall
Number (LRN) keep while completing the basic education program, regardless of
transfer to another school or learning center in the public or private
sector, and promoting/moving up to the secondary level. (DO 22, s.
2012).
Name of Learner Name of an Individual as reflected in the birth certificate or
equivalent document.
Sex (Male or Learner’s biological and physiological sex as reflected in the birth
Female) certificate or equivalent document.
Birth Date Date of birth of the learner as reflected in the birth certificate or
other equivalent document.
Age by June (1st The actual age in years of a learner as of last birthday by the 1 st
Friday) Friday of each school calendar year.
Mother Tongue The language first learned by a learner. There are 19 major
languages identified in the recent DepED orders which are:
Tagalog, Kapampangan, Pangasinense, Iloko, Bikol, Cebuano,
Hiligaynon, Waray, Tausog, Maguondanaoan, Maranao, Chabaca-
no, Ybanag, Ivatan, Sambal, Aklanon, Kinaray-a, Yakan, and
Surigaonon. (DO 16, s. 2012 and DO 28, s. 2013)
IP (Specify ethnic A group of people or homogenous societies identified by self-
group) ascription and ascription by others, who have continuously lived as
organized community on communally bounded and defined
territory.
Religion The religious belief of the learner. For Muslim learners, “Islam” will
be written instead of Muslim under this column.
House #/Street Refer to the place/location where the learner currently holds
residence.

Municipality
Province
Name of Father The name of father of the learner as reflected in the birth
certificate or equivalent document.
Name of Mother The name of mother of the leaner as reflected in the birth
certificate or equivalent document.
Name of Guardian The person who oversees the welfare of the learner in behalf of the
parents.
Relationship to The relationship between the learner and the guardian

Contact Number of Contact number of the parent/s or the guardian/s of the learner.

Performance Task 3:
Assisting the Cooperating Teacher in Accomplishing Required School
Form
As practice teacher, you might not be allowed to accomplish all the
forms yet, however, there might some instances that you are required to
assist in preparing some school forms. Make sure that you do it accurately
and legibly (if done manually) and encode properly (if done online). Review
all entries or have it checked by your Cooperating Teacher before you finally
submit. It is best to have a receiving copy (if done manually) and save the
file (if done online). Put a copy of the school form that you were requested to
do/accomplish and have it signed by your Cooperating Teacher. It is best if
you can get her comments/remarks and suggestions.

Learning Task 10: Building Networking and Linkages
Performance Task 1:
Assisting my Cooperating Teacher in Parent-Teacher Conference
A parent-teacher conference is a meeting between a student’s parents
and teacher or teachers to discuss the child’s academic performance and
other areas for improvement. This conference is held every quarter or may
be done as needed. Parents may also request for the conference especially if
there are matters to discuss to improve their children’s performance in
school. Your Cooperating teacher might request you to assist him/her in a
Parent-teacher conference. Confer with your CT and write down the things
that you need to prepare and the things you need to do.

Performance Task 2:
Providing supports by being volunteer tutors as part of auxiliary
service in partner school.
Aside from your teaching duties whether face-to-face or virtually, you
need to render auxiliary services. You may be requested to volunteer as a
tutor to students who need help to cope with their academic work. Record in
the logbook the details of what you have done to assist the students
assigned to you.

(CBAR)
Performance Task 1:
Conducting Classroom-Based Action Research (CBAR) an a specific
teaching-learning area
Using the action-research based model of the Department of Education
(DepED) DO 16 s. 2017, write an action research on your learning area. You
may ask on your Cooperating Teacher for possible research problems that
you can work on together. You may also want to revisit the action research
proposal that you have done in FS 1 and 2. There might be some areas that
you will need to explore or work on. It is best that you also see examples of
accomplished action research.

Performance Task 2:
Listing references used in CBARs following TEI prescribed
referencing or citation styles.
After you have chosen your topic, you are now ready to list down the
references that you may need in your action research. You may visit your
Online Public Access Catalog (OPAC). This is the online database of materials
in your library. It tells you the available materials needed in your research.
Try also to search other open online resources and other programs to enrich
your research.
Performance Task 3:
Sharing results of the research with an audience through any
available platform.
Now that you have completed your action research, you are now ready to
share the results. Your College Supervisor and Cooperating Teacher will
schedule the dissemination of your research together with the other interns
in an available learning platform. Dissemination of your research findings will
enable others to learn more from your research.
