Concept Note for Addressing Health Inequities

Background and Rationale

Health inequities are systematic, avoidable, and unjust disparities in health outcomes and access
to healthcare services experienced by different population groups. These disparities are often
rooted in social, economic, and environmental factors, leading to significant variations in health
status and access to care. Addressing health inequities is crucial for achieving overall health
equity and ensuring that all individuals have the opportunity to attain their full health potential.

Project Goal and Objectives

The goal of this project is to reduce health inequities and promote health equity in targeted
communities. The specific objectives are:

including economic, environmental, and cultural factors.

2. Improve access to quality healthcare services for marginalized populations through

targeted interventions and capacity building.

3. Empower communities to advocate for their health needs and participate in the
development of solutions.

4. Strengthen data collection and analysis on health outcomes and access to care by
demographic factors to better understand and address health inequities.

5. Foster collaboration across sectors to create comprehensive approaches to health

equity, including partnerships with education, housing, and social services.

developing solutions through participatory approaches.

3. Capacity building: Train healthcare providers and community health workers on

recognizing and addressing health inequities, including cultural competency training and
education on social determinants of health.

4. Targeted interventions: Implement evidence-based interventions to improve access to

healthcare services for marginalized populations, such as mobile clinics, telemedicine,
and community-based health programs.

5. Monitoring and evaluation: Establish a robust monitoring and evaluation framework to

track progress, measure impact, and inform program adjustments.
